超声造影对食管胃底静脉曲张的诊断价值

摘　　要：目的探讨超声造影在食管胃底静脉曲张诊断中的应用价值。方法根据胃镜检查结果,将93例上消化道出血患者分为观察组(食管胃底静脉曲张,48例)及对照组(无食管胃底静脉曲张,45例),采用SonoVue造影剂结合CPS造影成像技术行超声造影,观察两组患者食管下段双层黏膜和黏膜下层厚度、造影剂始增时间、峰值时间。结果观察组食管下段双层黏膜和黏膜下层厚度轻度患者达(5.98±0.54)mm,中重度患者达(8.13±0.45)mm,均大于对照组的(2.76±0.73)mm(P<0.05);造影剂始增时间轻度患者为(16.70±0.67)s,中重度患者(19.14±0.99)s,均大于对照组的(15.82±0.62)s(P<0.05);达峰时间轻度患者为(23.45±0.80)s,中重度患者为(25.41±1.23)s,均大于对照组的(22.69±0.67)s(P<0.05)。结论超声造影是一种方便的诊断手段,有望成为临床筛查食管胃底静脉曲张的一种新方法。Objective To evaluate the applications of contrast-enhanced ultrasound in the diagnosis of esophageal gastric varices.Methods A total of 93 patients were classified into two groups by gastroscopy：one was observation group including 48 patients with esophageal gastric varices and the other was control group including 45 patients without esophageal gastric varices.By contrast-enhanced ultrasound,the thickness of the lower esophageal bilayer mucosa and submucosa,AT and TP of two groups were observed and compared.Results In the observation group with less advanced esophageal gastric varices,the thickness of the lower esophageal bilayer mucosa and submucosa was（5.98 ± 0.54） mm,AT was（16.70 ± 0.67） s,and TP was（23.45 ±0.80） s.In the observation group with more advanced esophageal gastric varices,the thickness of the lower esophageal bilayer mucosa and submucosa was（8.13 ±0.45） mm,AT was（19.14 ±0.99） s,and TP was（25.41 ±1.23） s,which were significantly different from those of the control group（P 0.05）.Conclusion The contrast-enhanced ultrasound is a convenient diagnostic tool,which is expected to become a clinical new screening method for esophageal gastric varices.
