摘 要:目的探讨心肌应变率成像(SRI)技术对儿童川崎病心功能评估的应用价值。方法收集了本院2008~2009年川崎病患儿38例(川崎病组)和健康志愿者18例(正常组)。应用SRI技术分析左心室12节段收缩期及等容舒张期左心室心肌的应变率,并与正常组对比。分析所有患儿二维经胸超声心动图冠状动脉近段的表现。结果川崎病组左心室壁部分节段的收缩期峰值应变率、舒张期应变率低于正常组。Simpson法所测的川崎病患儿左心室整体射血分数与正常组比较差异有统计学意义(P<0.05)。超声心动图显示14例患儿冠状动脉扩张,3例患儿冠状动脉瘤形成。结论川崎病患儿的左心室局部收缩和舒张功能是受损的,SRI技术具有较高的敏感性,可评估川崎病患儿心肌收缩和舒张功能。Objective To assess heart function by strain rate imaging in children with Kawasaki disease.Methods A total of 38 subjects with Kawasaki disease were hospitalized from 2008 to 2009 and were retrospectively analyzed.Eighteen healthy subjects were enrolled.The systolic period and isovolumic relaxation of 12 segments were diagnosed by strain rate imaging.The features including two-dimension echocardiography were analyzed and compared.Results The systolice and diastolic strain rate were decreased significantly at part of the left ventricular wall in children with Kawasaki disease.Compared with normal control,ejection fraction measured by Simpson's method was decreased(P 0.05).Fourteen patients with coronary artery dilatation and 3 patients with coronary aneurysm were diagnosed by echocardiography.Conclusion The regional left ventricular systolic and diastolic functions in the children with Kawasaki disease were impaired.The left ventricular function in children with Kawasaki disease was assessed by new methods of strain rate imaging.The segment wall movement can be quantitatively determined by strain rate imaging,which is important in clinical evaluation of segmental function.
