改良Stark法阴式全子宫切除术术式研究

摘　　要：目的研究改良stark法阴式全子宫切除术临床效果及有无远近期并发症的发生。方法我们对120例实施改良stark法阴式全子宫切除术病人（研究组），与120例实施stark法阴式全子宫切除术病人（对照组）进行对照研究，观察两组病人术中，术后情况，并对所有病人进行术后3月、6月、2年随访。结果研究组除了手术时间比对照组稍微延长了5-10分钟，且有统计学意义（p〈0．05）外，术中出血量。术后住院天数，肛门排气时间，排便时间，拔尿管后自解小便时间及术后病率，均无统计学意义（p〉0．05）。术后3月、6月随访，两组均无近期并发症发生。术后2年随访，对照组有2例阴道前壁部分膨出，1例全部膨出，2例阴道顶端脱垂，复发率4．17％；研究组无1例复发，两组比较有统计学意义（p〈0．05）。结论改良stsrk法阴式全子宫切除术，具有手术过程简化，手术时间短，术中出血量少，肛门排气早，进食早，恢复快，住院天数少，费用低等优点，且无远近期并发症（指2年内）发生，值得临床推广。Objective To study the modified STARK vaginal hysterectomy and whether the clinical effect of the proximity of complications. Methods 120 cases of the implementation of our modified STARK vaginal hysterectomy（study group）, and 120 cases of the implementation of STARK vaginal hysterectomy （control group） , patients were observed the operation situation and after operation situation, and all patients were follow-up in 3-month, 6-month and 2-year. Results Study group＇s surgery were slightly longer than the extension of the 5-10 minutes, and there is statistical significance （p 〈0.05） , the blood loss volume, the number of days after hospitalization, anal exhaust time, defecation time, after the self-catheter urine out of time and postoperative morbidity, there was no statistical significance （p〉 0.05）. After 3-month, 6-month follow-up, no complications occurred in two groups. After 2-year follow-up, 2 cases in control group of vaginal prolapse anterior part. 1 eases of all the bulging, two cases of the top vaginal prolapse, recurrence rate 4.17%; 1 cases in study group without recurrence, two groups were statistically significant （p 〈0.05） . Conclusion The modified method STARK vaginal hysterectomy, with surgery to simplify the process. Shorter operation time, less bleeding, earlyer passage of gas by anus and eating. Rapid recovery, fewer days of hospitalization, and low cost, and far short-term complications （mean 2 years） , this surgery is worthy to be popularized.
