吸烟的高血压患者血液流变性和外周动脉超声变化

摘　　要：目的 了解吸烟埘高血压患者血液流变性及外周动脉粥样硬化发生的影响.方法 高血压吸烟组（n=32）;高血压非吸烟组（n=30）;不吸烟、且无血脂异常、糖尿病、心、脑血管疾病和肾脏疾病的人员作为正常对照组（n=30）.上述三组均测定血液流变学参数,并作颈动脉、下肢动脉彩色超声检查.结果 高血压各组低切变及高切变率下全血黏度、血浆黏度、血沉（ESR）、血沉方程K值、红细胞聚集指数、红细胞刚性指数、纤维蛋白原及血小板聚集率均显著高于正常对照组（P〈0.01）;且吸烟组上述参数显著高于非吸烟组（P〈0.05）.高血压各组颈动脉,下肢动脉的动脉粥样硬化超声检出率显著高于对照组（P〈0.01）.且吸烟组颈动脉、下肢动脉的动脉粥样硬化检出率显著高于非吸烟组（P〈0.01）.结论 高血压患者存在血液流变性异常;外周动脉粥样硬化的儿率增加,而吸烟则加重高血压患者的血液流变性异常,增加外周动脉粥样硬化发生的几率.Objective To find out affection of smoking on hemorhealogy and development of periphery artery atherosclerosis in patients with hypertension. Method Group of hypertension with smoking（n=32）;group of hypertension without smoking（n=30）,the persons without smoking,dyslipidemia,diabetes,hypertension,cardiov- ascular, cerebral vascular disease and renal disease were served as normal control group（n=30）.Hemorheological parameters were determined and color ultrasound examination of carotid artery and low extremities artery were carried in all three groups. Results The blood viscosity at low shear rate（10s-1） and high shear rate （150s-1）,plasma viscosity, erythrocyte sedimentation rate（ESR）,ESR equation K value,erythrocyte aggregation index,erythrocyte rigidity index,plasma fibrinogen,and platelet aggregation rate were significantly higher in every group of hypertensive patients than of control（P〈0.01）.The above parameters were significantly higher in group of hypertensive patients with smoking than of hypertensive patients without smoking（P〈0.05）.Detection rates of carotid and low extremities atherosclerosis with color ultrasound examination were significantly higher in every group of hypertension than of control（P〈O.O!）.The detection rates were significantly higher in group of hypertension with smoking than of hypertensive patients without smoking（P〈0.01）. Conclusion There were abnormities of hemorheology and increase of atherosclerosis development ratio of peripheral artery in hypertensive patients.Smoking augmented abnormities of hemorheology and development ratio of peripheral artery atherosclerosis in hvoertensive oatients.
