肿瘤坏死因子α-308 A>G位点单核苷酸多态性与原发性胆汁性肝硬化易患性的Meta分析

摘　　要：目的探讨肿瘤坏死因子α(TNF-α)在-308 A>G位点单核苷酸多态性与原发性胆汁性肝硬化(PBC)易患性的关系。方法计算机检索MEDLINE、EMBASE、CBMdisc数据库,手工检索相关杂志。由两名评价员独立检索和提取资料并交叉核对,纳入相关病例对照试验。采用Cochrane协作网提供的RevMan 5.0.24软件进行Meta分析。结果共纳入病例对照试验6篇,共包括3 240例患者,其中病例组1 561例,对照组1 679例。Meta分析结果发现,-308 A>G TNF-α基因多态性与PBC易患性无明显关联,-308 A>G等位基因均不是PBC的危险因素(P>0.05)。剔除一项研究权重过大的文献后,敏感性分析结果与原始结果基本一致,进一步证实了原始结果的真实性。由于各研究存在明显异质性,遂根据基因型不同进行亚组分析,发现-308 GG、AA和AG基因型均不是PBC的易患性因素〔OR=0.88,95%CI(0.46,1.66),P=0.69;OR=0.95,95%CI(0.72,1.24),P=0.68;OR=1.19,95%CI(0.86,1.66),P=0.30〕。结论 TNF-α-308 A>G位点等位基因不是PBC的易患性因素。Objective To investigate the relationship between TNF - α - 308 A 〉 G polymorphism and susceptibility to primary biliary cirrhosis （PBC）. Methods Medline, EMBASE and CBMdisc database were searched in PC. Related jour- nals were also searched manually. Data were searched, retrieved and cross - checked by two assessors, and the data were in- volved in related case - control study. Data analysis was conducted by RevMan 5.0.24 software which was provided by Cochrane. Results Six case -control studies with a total of 3 240 PBC cases （1 561 in study group and 1 679 in control group） were in- cluded. Meta - analysis results showed that there was no significant association between TNF - α - 308 A 〉 G polymorphism and susceptibility to PBC, and - 308 A 〉 G was not a risk factor for PBC （ P 〉 0. 05 ）. Sensitivity analysis was performed after an o- mission of one study with the most weight. The results in all individuals＇ analyses and subgroup analyses were not influenced excessively by omitting this study. In the subgroup analysis based on genotypes, -308 GG, AA and AG were not a risk factor for PBC （OR=0.88, 95%CI （0.46, 1.66）, P=0.69; OR=0.95, 95%CI （0.72, 1.24）, P=0.68; OR=1.19, 95%CI （0. 86, 1.66） , P = 0. 301. Conclusion TNF-α- 308 A 〉 G polymorphism was not a risk factor for PBC.
