摘  要:目的: 探讨三维CT 成像在颅内病变中的临床应用价值。材料与方法: 对71 例颅内病变患者行螺旋CT 增强扫描,然后对颅内血管和/ 或病变进行三维重建。其中23 例行CT 血管造影(CTA) 扫描( 延时20 秒) ,48 例非CTA 扫描( 延时90 秒) 。结果: (1)23 例CTA 扫描者,血管表面遮盖法(SSD) 与最大强度投影法( MIP) 重建都能显示颅内动脉的第3 级分支。SSD 对显示血管的立体空间关系比较直观;MIP 图像类似DSA 图像。(2) 33 例颅内病变SSD 能清楚显示病变与血管和颅骨的立体解剖全貌,但失去有利于病变定性诊断的密度CT 征象。33 例多平面重建( MPR) 有利于对病灶从多方向观察,保留有助于定性诊断的密度CT 征象,但缺乏病变与周围解剖结构的立体观。结论: 3 种三维重建相结合可增加对颅内病变与血管和周围组织结构间的立体解剖信息,使神经外科医生在手术前对病灶有更全面的了解,有助于术前对手术计划的制定。Objective: To assess the clinical application of CT 3 D reconstruction for intracranial diseases.Materials and Methods:Helical CT scanning and 3 D reconstruction of intracranial vessels and/or lesions were performed in 71 cases with intracranial diseases. CTA scanning with 20s delay was used in 23 cases, while non CTA scanning with 90s delay in 48 cases.Results:(1) For all 23 cases using CTA, both SSD and MIP showed the intracranial arteries up to the third grade. SSD stereoscopically displayed the relationship of the arteries, while MIP image looked similar to DSA image. (2) In 33 cases with intracranial lesions, SSD stereoscopically demonstrated the anatomic relationship between lesions, arteries and skull, though it lost some diagnostic features, such as characters in density. MPR could show the lesion in any desired orientation and keep the density features, helpful for diagnosis, but the image was lack of stereoscopical view of the anatomic relationship between the lesion and surrounding structures.Conclusion: The combination of three 3 D reconstruction images can offer more information about the 3 D anatomy between the lesions, vessels and surrounding structures, providing neurosurgeons with more clear anatomic picture of the lesion and helping them to plan the treatment.
