正常椎体的磁共振化学位移成像技术定量分析

摘  要:目的使用磁共振化学位移成像技术测量正常椎体骨髓信号强度(SI)下降指数。方法 35例共238个正常椎体进行常规MRI扫描和同相位、反相位成像检查,在每一个正常椎体上设置感兴趣区(ROI),在同相位及反相位图像的同一位置分别测量其信号强度值,计算椎体信号强度下降指数,SI下降指数=[(SI同相位-SI反相位)/SI同相位]×100%,并将颈椎、胸椎与腰椎之间进行两两比较,同时将椎体信号强度下降指数与年龄进行相关性分析。结果与同相位图像相比较,所有的椎体在反相位图像上均显示出信号下降,全部椎体平均信号强度下降指数为(71.1±10.2)%,其中颈椎78.2%、胸椎72.3%、腰椎68.8%,颈、胸、腰椎间信号强度下降指数间没有统计学差异,信号强度下降指数与年龄间呈正相关性。结论在化学位移成像图像上,正常椎体信号强度下降指数会呈现出一定程度的变化。Objective To establish retrospectively a range of values for signal intensity change in normal vertebral marrow by using chemical shift magnetic resonance (MR)imaging. Methods A total of 238 normal vertebrae in 35 patients were studied by using 1.5-T chemical shift MR imaging. The proportional change in signal intensity on in-phase compared with out-of-phase images was calculated .This change in signal intensity (expressed as a percentage) was compared. Results All vertebral levels exhibited a decrease in signal intensity on out-of phase images compared with in-phase images; no increases in signal intensity were noted. The mean decrease in signal intensity(expressed as a percent) for all vertebral levels was (71.1±10.2)%. According to vertebral level, the mean decrease in signal intensity was78.2% for cervical vertebrae, 72.3%for thoracic vertebrae, and 68.8%for lumbosacral vertebrae; there was no significant variability among the three main(cervical, thoracic, or lumbosacral) spinal segments. There was correlation between decreases in signal intensity and age. Conclusion There was some variability in the loss of signal intensity according to location(spinal segment), and there were associations between decreases in signal intensity and age.
