摘  要:目的:观察氨茶碱对重症胎粪吸入新生兔的肿瘤坏死因子-α(TNF-α)和白细胞介素-8(IL-8)的影响。方法:将24只新生兔随机分成灌入胎粪后实施同步间歇指令通气(SIMV)应用氨茶碱组(氨茶碱组,n=6)、灌入胎粪后实施同步间歇指令通气组(氨茶碱对照组,n=6)、胎粪灌入后不予通气组(通气对照组,n=6)以及灌生理盐水不予通气组(空白组,n=6)。氨茶碱对照组和氨茶碱组给予机械通气6 h。氨茶碱组在胎粪灌注后0.5 h和2.5 h后静脉给予氨茶碱(2.0 mg/kg),并用酶联免疫吸附试验(ELISA法)测定肺组织匀浆液及支气管肺泡灌洗液(BALF)中TNF-α和IL-8含量。结果:氨茶碱组和氨茶碱对照组比较肺组织匀浆和BALF中TNF-α和IL-8含量降低,差异均有统计学意义(均P<0.05)。与通气对照组、空白组比较,氨茶碱对照组、氨茶碱组的肺组织匀浆和BALF中TNF-α和IL-8含量明显升高,差异均有统计学意义(均P<0.05)。结论:氨茶碱可以降低新生兔重症胎粪吸入肺组织匀浆和BALF中TNF-α和IL-8的水平。Objective:To observe effect Aminophylline on the levels of the tumor necrosis factor-α(TNF-α) and interleukin-8(IL-8) of newborn rabbits with severe meconium aspiration.Methods:24 newborn rabbits were randomly divided into four groups:implement synchronized intermittent mandatory ventilation(SIMV) after meconium instillation treated with Aminophylline(Aminophylline group,n=6),implement synchronized intermittent mandatory ventilation after meconium instillation(Aminophylline control group,n=6),no ventilation after meconium instillation(ventilation control group,n=6) and the saline irrigation with no ventilation group(control group,n=6).Aminophylline control group and Aminophylline group received 6 hours of mechanical ventilation.Rabbits intravenously received Aminophylline(2.0 mg/kg) at two doses 0.5 h and 2.5 h after meconium instillation in Aminophylline group.Enzyme-linked immunosorbent assay(ELISA) was used to measure TNF-α and IL-8 in lung homogenates and bronchoalveolar lavage fluid(BALF).Results:Compared with Aminophylline control group,TNF-α and IL-8 in lung homogenates and BALF in Aminophylline group were lower,the differences all were statistically significant(all P0.05).Compared with ventilation control group and the control group,TNF-α and IL-8 in lung homogenate and BALF in Aminophylline group and Aminophylline control group were higher,the differences all were statistically significant(all P0.05).Conclusion:Aminophylline can reduce the levels of TNF-α and IL-8 in newborn rabbits with severe meconium aspiration.
