摘  要:目的对昆明市一起甲型副伤寒的流行进行调查,以了解其流行病学和临床发病特点。方法从2008年9月22日~11月28日,收集105例发热患者的临床资料,均为昆明市某职业技术学院师生。采用自制《发热病人个案调查表》行流行病学调查和临床特点分析。结果 78例血培养发现甲型副伤寒杆菌生长,主要症状为发热(100%)、相对缓脉(95.2%);4例合并伤寒肝炎,5例合并支气管肺炎。经敏感抗生素治疗后,全部105例患者治愈出院,未发生肠穿孔及肠出血等严重并发症。其中复发2例,经治疗后治愈出院。结论甲型副伤寒除发热及相对缓脉外,症状多不典型,并发症不多且较轻,对多种头孢类抗生素及三代氟喹诺酮药物敏感,治疗效果良好。Objective To investigate and analyze the epidemiological and clinical characteristics of an outbreak of paratyphoid A in the eastern suburb of Kunming City. Methods The clinical data of 105 fever patients who were teachers/students in a professional technology college in Kunming City were collected from September 22th, 2008 to November 30th, 2008. Their epidemiological and clinical characteristics were investigated and analyzed by a self - made Case Investigation Questionnaire for Fever Patients. Results Blood culture indicated that bacterium paratyphosum A grew in 78 cases. Their cardinal symptoms were fever (100%) and relative infrequent pulse ( 95. 2% ). There were 4 cases complicated with typhoid hepatitis, and 5 cases with alveobronchiolitis. After treated with sensitive antibiotics, all of 105 patients were healed and discharged from hospital without any severe complications such as enterobrosis and enterorrhagia. Only 2 recurrent cases were found, and hoth left hospital after effective treatment. Conclusion The symptoms of paratyphoid A appear to be mostly atypical except fever and relative infrequent pulse. It is of rare and mild complications,and sensitive to a variety of cephalosporin and the third generation of fluorine quinolone.
