胸部局限型Castleman病的CT诊断

摘　　要：目的:探讨胸部局限型Castleman病的临床、病理及CT表现。方法:回顾性分析10例经手术病理证实的胸部局限型Castleman病的病例资料。结果:10例患者中,病变位于前纵隔1例,后纵隔3例,中纵隔5例,左侧斜裂1例。CT表现:左侧斜裂病变平扫示中央区不规则钙化,增强呈不均匀中度强化;余9例病变平扫均呈均匀等密度影,其中8例增强呈较均匀显著强化,另1例位于肺门区病变呈轻度强化。病理表现:透明血管型9例,镜下示滤泡生发中心形成,滤泡间大量增生的毛细血管,透明血管变性;浆细胞型1例,镜下示滤泡生发中心正常或增大,滤泡间大量浆细胞浸润。结论:胸部局限型Castleman病的临床、CT表现与病理密切相关,结合较有特征性的临床及CT表现有利于该病术前诊断。Objective：To investigate the clinical features,pathology and CT findings of localized Castleman′s disease of chest.Methods：Data of 10 cases with histologically proved localized Castleman′s disease of chest were analyed retrospectively.Results：Of 10 cases,1 case was located in the anterior mediastinum,3 cases in the posterior mediastinum,5 cases in the middle mediastinum,and 1 case in the left oblique fissure.CT findings：On CT plain and enhanced images,central branching calcification and heterogeneous moderate enhancement was respectively observed in the lesion located in the left oblique fissure.Mass with homogeneous density was observed on the CT plain images of the other 9 cases.On CT enhanced images,8 of the 9 cases lesions showed homogeneous intensive enhancement,and another one located in the right hilum showed mild enhancement.Pathological findings：9 cases were of hyaline vascular type,their pathological features being follicular germinal center formation,large numbers of hyperplasia of capillary among follicles,and transparent vascular denaturation.Another one was of plasma cell type,pathological features included normal or increscent follicular germinal center,numerous plasma cell infiltration among follicles.Conclusion：There is a close correlation among clinical festures,CT findings and pathology of localized Castleman′s disease of chest.It is conducive to preoperative diagnosis to combine characteristic CT findings with clinical features.
