摘 要:目的:探讨系统性自我管理教育对社区2型糖尿病患者生命质量及焦虑抑郁情绪的影响。方法:2009年2月~2010年7月随机选取长沙市10个社区,干预组与对照组各5个,共纳入2型糖尿病患者248人为研究对象,干预组予以系统性自我管理教育,对照组予以常规社区教育。采用中文版糖尿病特异性生命质量测定量表(A-DQOL)、焦虑自评量表(SAS)、抑郁自评量表(SDS)对干预前后两组患者生命质量、焦虑、抑郁情绪进行评定。结果:干预前,两组糖尿病患者生命质量各维度、焦虑及抑郁情绪检出率差异均无统计学意义(P〉0.05);干预后,差异均有统计学意义(P〈0.01)。干预后,干预组生命质量各维度较对照组明显改善(P〈0.01);干预组焦虑检出率较对照组明显下降(8.94%vs.44.4%,P〈0.01);干预组抑郁检出率较对照组明显下降(23.58%vs.56.00%,P〈0.01)。结论:系统性自我管理教育能有效改善社区2型糖尿病患者生命质量及焦虑、抑郁情绪,值得进一步推广。Objective To explore the effect of systematic self-management education on quality of life,anxiety and depression of patients with Type 2 diabetes in communities.Methods A total of 248 patients with Type 2 diabetes from 10 communities of Changsha from February 2009 to July 2010 were randomized into an intervention group and a control group.The intervention group received systematic self-management education,while the controls received routine community education.The quality of life,anxiety and depression were measured by adjusted diabetes-specific quality of life scale(A-DQOL),self-rating anxiety scale(SAS) and self-rating depression scale(SDS) respectively.Results The quality of life,morbidity of anxiety or depression of the intervention and the control group were comparable at the baseline(P0.05).One and half year later,there was significant difference between the two groups(P0.01).Compared with the control group,the intervention group got better quality of life(P0.01),less morbidity of anxiety(8.94% vs.44.4%,P0.01) and depression(23.58% vs.56.00%,P0.01).Conclusion Systematic self-management education can effectively improve the quality of life,reduce the level of anxiety and depression of patients with Type 2 diabetes in communities,which deserves further generalization.
