摘 要:目的:探讨不同出生体重早产新生儿早期营养对出生后两周内体重变化的影响。方法:采用定群研究和前瞻性观察方法对2007年10月~2008年10月入住某省立儿童医院新生儿科的早产儿能量、蛋白质摄入与体重增长情况进行评估。按照出生体重不同,将符合条件的177例研究对象分为体重<2 000 g组和体重≥2 000 g组,比较两组能量-蛋白质摄入对体重增长的影响。结果:体重<2 000 g组和体重≥2 000 g组出生后第1周内平均每天每1 000 g总能量、蛋白质的摄入量差异没有统计学意义,第2周有统计学意义(t分别为-2.571、-5.390,均P<0.05)。体重<2 000 g组恢复出生体重日龄和体重下降最大幅度明显高于体重≥2 000 g组(t分别为1.367、2.426,均P<0.05),经绘制曲线图发现,2周内,体重≥2 000 g组体重增长值均高于体重<2 000 g组。结论:两组在住院期间2周内能量和蛋白质摄入普遍不足,均未能达到理论要求。早产儿,尤其是出生体重较低者应该尽早进行合理的营养,补足能量-蛋白质需求。Objective: To explore the effect of early nutrition on weight changes for two weeks in premature neonatus of different birth weight. Methods: 177 premature neonatus were collected in Anhui Provincial Children Hospital from October 2007 to October 2008. The relationship between the intake of protein - energy and weight growth were analyzed by cohort study and prospective dynamic observation method. According to birth weight, 177 premature neonatus were divided into two groups, group 1 〈2 000 g and group 2≥2 000 g. Theeffect about the intake of energy - protein and weight growth between two groups were analyzed. Results: There was no statistical significance about the intake of energy - protein in the first week between two groups, while the difference of intake of energy - protein was significant in the second week (t = - 2. 571, - 5. 390 respectively, both P 〈 0.05) . The extent of weight loss and days when the preterm infant recovered birth weight in the birth weight 〈 2000g group were higher than the birth weight ≥2 000 g group ( t = 1. 367, 2. 426 respectively, both P 〈0. 05) . By drawing graph, we found that the value of weight growth in the birth weight ≥2 000g group was higher than the birth weight 〈 2 000 g group for two weeks. Conclusion : Energy - protein intake was generally deficient in both groups during hospital in two weeks and didnt achieve to the theory demand. Preterm infant especially low birth weight infant should be provided nutritional requirement pattern to meet with sufficient energy and protein as soon as possible.
