比索洛尔联用贝那普利对慢性心力衰竭患者左室重构和心功能的影响

摘　　要：目的 评价比索洛尔与贝那普利治疗慢性充血性心力衰竭（CHF）的临床疗效.方法 将285例CHF患者随机分为3组（均给予常规强心、利尿、扩血管治疗）：比索洛尔治疗组102例,贝那普利治疗组75例,比索洛尔与贝那普利组108例,观察三组治疗前后SBP、DBP、心率（HR）、左心室收缩末内径（LVESd）、左心室舒张末内径（LVEDd）及左心室射血分数（LVEF）的变化.结果 联合治疗组显效率和总有效率（75.3%和96.8%）均高于比索洛尔组（59.3%和82.7%,P＜0.05）和贝那普利组（61.1%和81.9%,P＜0.05）,无效率（3.2%）显著低于比索洛尔组（17.3%,P＜0.01）和贝那普利组（18.1%,P＜0.01）.治疗前后三组患者HR、SBP及DBP均有改善（P＜0.05）,联合治疗组较另外两组改善更明显（P＜0.01）.治疗前后三组LVEDd、LVESd及LVEF均显著改善（P＜0.05）,联合治疗组较另外两组改善明显.结论 比索洛尔治疗CHF的临床疗效与贝那普利相当,两者联合应用与单独使用相比,前者不仅能更好地提高疗效,而且安全有效.Objective To explore the clinical efficacy of bisoprolol combined with Benazepril on chronic congestive heart failure. Methods 285 cases of CHF were randomly divided into 3 groups： 102 patients treated with bisoprolol, benazepril 75 patients, bisoprolol combined with benazepril group of 108 cases. Observed three groups changes of SBP, DBP, heart rate, left ventrieular end-systolic diameter （LVESd）, left ventricular end di- astolic diameter （LVEDd） and left ventricular ejection fraction （LVEF）. Results The efficiency and total effi- ciency （75.3% and 96.8%） in combined treatment group were higher than bisoprolol group （59.3% and 82.7%, P〈0.05） and the benazepril group （61.1% and 81.9%, P〈0.05）, inefficient （3.2%） was significantly lower than the bisoprolol group （17.3%, P〈0.O1 ） and the benazepril group （18.1%, P〈0.O1 ）. Three groups of patients be- fore and after treatment heart rate, SBP and DBP were improved （P〈0.05）, combined treatment group improved more significantly than the other two groups （P〈0.01）. Three groups before and after treatment LVEDd, LVESd and LVEF were significantly improved （P〈O.05）, combined treatment group improved significantly than in the oth- er two groups. Condusion Administration of bisoprolol plus benazepfil can significantly improve the clinical symptoms and cardiac function in patients with CHF,which is mole effective than the single benazepril or benazepril on the treatment. It can improve the prognosis and be used for the routine treatment of CHF.
