以家庭为单位自我管理模式对脑卒中患者康复效果的影响

摘　　要：目的:研究对脑卒中患者实施以家庭为单位的综合干预是否对提高患者生存质量产生影响。方法:将铁营医院铁营社区101位脑卒中患者随机分为实验组、对照组。实验组51名,对照组50名。对实验组的患者采用以家庭为单位自我管理的康复护理模式进行社区护理干预。对对照组的患者采用常规社区护理管理模式。分别于干预前后使用家庭功能量表、生存质量量表(SIS)对实验组、对照组进行评价。结果:实验组和对照组脑卒中患者的基本情况具有可比性(P>0.05),实验组家庭功能除在"得到及时帮助和对困难给予支持"两方面无差别外,其他关系间的差异有统计学意义;对照组家庭功能改善差异无统计学意义。实验组生存质量在干预前后有统计学差异(P<0.01);对照组仅在力气维度和参与维度的生存质量差异有统计学意义(P<0.05),其他维度的生存质量差异均无统计学意义(P>0.05)。结论:脑卒中社区护理与以家庭为单位自我管理模式综合干预是改善患者家庭功能及生存质量的有效方法。Objective： To explore the effect of self-management mode on rehabilitation of patients with cerebral stroke in community nursing. Methods： Totally 101 patients with cerebral stroke lived in Tieying community were divided into two groups randomly （51 in experimental group and 50 in control group）. Patients in the control group were cared with routine nursing method, while patients in the experimental group were cared using self-management rehabilitation mode based on family as a vital unit. Family function and quality of life were evaluated before and three months after the intervention. Results： The basic information of the two group patients were comparable （P0.05）. Patients in the experimental group improved their household function except ＂getting immediate help and support in difficulties＂, while patients in the control group didn＇t improve their household function. Patients in the experimental group improved their quality of life （P0.01） while patients in the control group didn＇t improve their quality of life except the strength and participation dimensions （P0.05）. Conclusion： The self-management mode based on family as a vital unit is an effective method in improving the household function and the quality of life of the patients with cerebral stroke.
