摘  要:目的:本研究利用MR扩散张量成像部分各向异性(FA)值,研究老年人的不同水平认知功能和脑白质异常的相关性,并探讨脑内微静脉改变的意义。方法:正常认知功能(CN)组13例,轻度认知功能障碍(MCI)组12例,痴呆组10例。采用简易智能状态量表(MMSE)、临床痴呆量表(CDR)、总体衰退量表(GDS)和日常生活活动能力量表(ADL)进行分组。在双侧额叶、顶叶、枕叶、颞叶、侧脑室旁以及胼胝体膝部和压部白质选取感兴趣区。组间比较使用ANOVA方差分析,FA值与神经心理量表评分之间进行Pearson相关分析。结果:①双侧扣带束后部的FA值降低在三个组间有显著差异,痴呆组左侧颞叶白质的FA值降低与CN组和MCI组有显著差异。②以双侧扣带束后部、左侧颞叶和顶叶为主等区域的FA值和MMSE,CDR,GDS和ADL相关。③观察到认知功能障碍者双侧脑室旁白质内微静脉显示稀疏。结论:老年人的认知功能减退和白质异常存在一定相关性,以扣带回后部、颞叶和顶叶的白质改变较显著。Purpose:To investigate the relationship between fractional anisotropy(FA) value of diffusion tensor imaging(DTI) and cognitive function in elders,and observe their microvascular changes in white matter.Methods:Subjects were grouped into normal cognition(CN)(13),mild cognitive impairment(MCI)(12) and dementia(10).Neuropsychology tests included mini-mental state examination(MMSE),clinical dementia rating(CDR),global deterioration scale(GDS) and activity of daily living scale(ADL).Total 12 regions of interest were set at bilateral frontal,parietal,occipital and temporal white matter,bilateral corona radiata,genu and splenium of corpus callosum respectively.ANOVA analysis was used for comparison between groups.Pearson correlation analysis was done between FA values and neuropsychology test scores.Results:①Difference of FA in bilateral posterior cingulum fibers was statistically significant among three groups.Difference of FA in left temporal was statistically significant between dementia group and groups of MCI and CN.②FA of bilateral posterior cingulum fibers,temporal and parietal white matter were correlated with scores of MMSE,CDR,GDS and ADL.③White matter microvasculature changes were observed in cognitive impairment groups.Conclusion:There is a correlation between FA decrease and cognitive function decline in aging group.White matter changes are more obvious in the posterior cingulum,temporal and parietal structures.
