摘 要:目的:分析急性尿路梗阻性肾周炎的影像学特征、解剖以及病理生理基础,加深对该病认识。方法:回顾性分析134例梗阻性肾炎CTU影像表现,着重观察肾周筋膜毗邻关系和肾周间隙的受累情况及其CT分级。结果:134例梗阻性肾炎中,左侧结石72例,占53.7%,右侧结石55例,占41%,两侧结石7例,占5.3%。肾周筋膜增厚6例,渗出或积液在肾旁前间隙46例,肾周间隙97例,肾旁后间隙54例。肾周受累的CT分级:A级4.5%(6/134),B级17.2%(23/134),C级78.4%(105/134)。结论:熟悉腹膜后间隙的解剖结构,掌握梗阻性肾周炎病理情况下影像学表现,可以预测病变扩散途径和渗出程度,为临床诊断和治疗提供更多的影像学依据。Purpose: To well understand the acute obstructive perinephritis through the study of imaging feature and anatomic and pathophysiological basis.Methods:CT imaging features of 134 cases were retrospective studied,which were focused on the correlation with the renal fasciae,and the incidence of the perirenal space involvement in acute obstructive perinephritis.Results: In this group of acute obstructive perinephritis,calculi were found in the left(72/134),right(55/134) and bilateral(7/134)urinary tract.Our study showed that perirenal fascia thickening was displayed only in 6 cases,effusion in the anterior pararenal space in 46,in the pararenal space in 97,and posterior pararenal space in 54.The incidence of the perirenal space involvement was Grade A 4.5%(6/134),Grade B 17.2%(23/134),Grade C 78.4%(105/134).Conclusion: To be familiar with the anatomical structure of retroperitoneal space,and know well the imaging findings of obstructive perinephritis under the pathological situations,we can predict pathological dispersion and exudate extent,and provide more information for clinical diagnosis and treatment.
