精氨酸对急腹症术后患者免疫功能的促进作用

摘　　要：目的研究利用精氨酸强化的肠外营养对急腹症术后患者免疫功能的影响。方法选择我院住院的急腹症患者54例,随机分为研究组和对照组,其中研究组采用精氨酸强化的肠外营养,而对照组应用常规肠外营养。两组术后均连续应用肠外营养3 d。分别于术前1 d、术后1 d、术后4 d取血,测定血浆白蛋白、总蛋白、IgG、IgA、IgM、C3、C4、CD4^+、CD8^+等。结果研究组血浆白蛋白、总蛋白、CD4^+、CD8^+及CD4^+/CD8^+水平高于对照组,差异有统计学意义(P<0.05)。研究组在术后第4天C3、C4水平高于术前1 d,差异有统计学意义(P<0.05)。其他指标在两组之间差异无统计学意义(P>0.05)。结论精氨酸强化的肠外营养能促进急腹症术后患者的血浆白蛋白、总蛋白的恢复,提高免疫功能。Objective To study the effect of arginine on promoting the immune function in patients with acute abdomen after operation as the component of total parenteral nutrition. Methods A total of 54 inpatients with acute abdomen were divided into two groups ： the study group was treated with TPN plus arginine, and the control group was treated with routine TPN, both for 3 consecutive days, Blood was taken on day 1 before the operation, and on days 1, 4 after the operation. The plasma albumin, total protein, IgG, IgA, IgM, C3, C4, CD4 ＋ and CD8 ＋ were determined. Results There were statistically significant difference between the two groups in levels of plasma albumin, total protein, CD4＋ ,CD8 ＋ and CD4 ＋/CD8 ＋ , but there was no significant difference in IgG, IgA, and IgM. Conclusions Arginine - enriched TPN can promote the recovery of plasma albumin, and total protein and strengthen the immune function.
