血小板参数在强直性脊柱炎治疗效果评价中的作用研究

摘　　要：目的研究血小板参数,包括血小板计数(PLT)和平均血小板体积(MPV)与强直性脊柱炎(AS)疾病活动的相关性,探讨其可能的机制。方法测定AS患者治疗前后PLT、MPV,以及急性时相反应物,包括血沉(ESR)、C反应蛋白(CRP)等指标并评估BASDAI和BASFI,比较各指标治疗前后的变化以及各指标之间的相关性,进行统计分析。结果 AS患者治疗后PLT、ESR、CRP测定值及BASDAI和BASFI显著低于治疗前(P<0.05),MPV测定值显著高于治疗前(P<0.05)。治疗前PLT升高组AS患者PLT、ESR、CRP、BASDAI、BASFI等指标均显著高于PLT正常组AS患者(P<0.05),MPV显著低于PLT正常组AS患者(P<0.05)。相关性分析表明PLT和BASDAI、BASFI呈正相关(P<0.05),MPV和BASDAI、BASFI呈负相关(P<0.05)。结论血小板参数PLT、MPV与AS病情活动有关,可作为判断AS疾病活动度的指标。Objective To investigate the correlation between disease activity of ankylosing spondylitis（AS）and platelet-associated parameters,including platelet count（PLT）and mean platelet volume（MPV）.Methods Acute phase reactants（APR）,including erythrocyte sedimentation rate（ESR）and C-reactive protein（CRP）,along with PLT and MPV were determined and compared before and after AS treatment.Meanwhile,BASDAI and BASFI were evaluated for the correlation analysis to the platelet-associated parameters and APR.Results PLT,ESR,CRP,BASDAI,and BASFI were significantly decreased post-treatment（P〈0.05）,MPV was significantly increased post-treatment（P〈0.05）.PLT,ESR,CRP,BASDAI,and BASFI were higher,whereas MPV was lower in PLT elevated AS patients prior to treatment.Besides,PLT was positively correlated to BASDAI and BASFI（P〈0.05）,MPV was negatively correlated to BASDAI and BASFI（P〈0.05）.Conclusion PLT and MPV are correlated to AS disease activity,therefore,PLT and MPV may serve as reference parameters of AS disease activity evaluation.
