1026例育龄妇女HPV感染情况调查及分析

摘　　要：目的:了解厦门市思明区已婚育龄期妇女HPV感染情况,HPV感染者宫颈细胞DNA定量异常情况,HPV感染者宫颈细胞学检查异常情况。方法:由麦克奥迪病理诊断中心对1 026例育龄妇女的宫颈细胞进行DNA定量分析及宫颈细胞TCT检查,采用TBS分类报告,采用凯普HPV分型诊断系统进行HPV核酸分析。结果:HPV感染人数119例,感染率11.6%,高危型HPV感染105例,感染率10.2%,低危型HPV感染14例,感染率1.4%,以高危型HPV感染为主,同时感染两种及以上高、低危HPV型别者19例,感染率1.9%,均存在高危型HPV感染。1 026例中,宫颈细胞DNA定量分析异常者54例,异常率5.3%,119例HPV感染者,25例存在宫颈细胞DNA定量分析异常,异常率21.0%,均为高危型HPV感染,907例无HPV感染者,29例存在宫颈细胞DNA定量分析异常,发生率3.2%,明显低于HPV感染者,两者差异有统计学意义。1 026例中,TCT检查异常26例,异常率2.5%,119例HPV感染中17例存在TCT检查异常,异常率14.3%,而907例无HPV感染者,TCT检查异常9例,异常率1.0%,HPV感染者TCT异常率明显高于无HPV感染者,差异有统计学意义。结论:已婚育龄期妇女HPV感染率11.6%,以高危型HPV感染为主;部分妇女存在两种型别以上HPV混合感染;HPV感染者宫颈细胞DNA定量异常率明显高于无HPV感染者,高危型HPV感染率明显高于低危型HPV感染者,宫颈细胞染色体DNA定量异常,宫颈液基细胞学检查可无异常。HPV感染者细胞学检查异常率明显高于无HPV感染者,30岁以上妇女宫颈癌筛查最佳的筛查方法是HPV检查联合宫颈细胞学检查。Objective：To understand the HPV infection status of married women of child-bearing age,abnormal conditions of DNA quantification in cervical cells and cervical cytological test in HPV-infected patients. Methods：The cervical cell samples of 1 026 women of child-bearing age received DNA quantification analysis and cervical cytological test,TBS was used in the study,Cape HPV genotype diagnosis system was used for HPV DNA analysis. Results：119 women were infected by HPV,the infection rate was 11.6%,including 105 women infected by high risk HPV（10.2%） and 14 women infected by low risk HPV（1.4%）,high risk HPV infection was the main infection type,19 women were infected by two or more than two kinds of HPV subtypes,the infection rate was 1.9%,high risk HPV infection existed;among 1 026 women,54 women were found with abnormal DNA quantification,the rate was 5.3%;among 119 HPV-infected women,25 women were found with abnormal DNA quantification,the rate was 21.0%,all the infection types were high risk HPV;among 907 women without HPV infection,29 women were found with abnormal DNA quantification,the rate was 3.2%,which was significantly lower than that of HPV-infected women;among 1 026 women,26 women were found with abnormal TCT results,the abnormal rate was 2.5%;among 119 HPV-infected women,17 women were found with abnormal TCT results,the abnormal rate was 14.3%;among 907 women without HPV infection,9 women were found with abnormal TCT results,the abnormal rate was 1.0%,which was significantly lower than that of HPV-infected women. Conclusion：The HPV infection rate of women of child-bearing age is 11.6%,high risk HPV infection is the main infection type;partial women are infected by more than two kinds of HPV subtypes;the abnormal rate of DNA quantification in HPV-infected women is significantly higher than that of the women without HPV infection,the infection rate of high risk HPV is significantly higher than that of low risk HPV,the women with abnormal DNA quantification may have normal TCT results;the a
