摘  要:目的:研究乙型肝炎病毒宫内感染TLR-4、HBD-2及IFN-γ水平及相关性。方法:选择乙肝标志物HB-sAg、HBeAg为阳性且肝功能正常的孕妇57例为试验组,根据脐血清乙肝两对半结果,分为宫内感染组26例,宫内未感染组31例,外周血血清HBV标志物均为阴性的正常分娩孕妇20例为对照组,采用ELISA法检测孕妇外周血血清IFN-γ水平;SYBRGreenⅠ实时荧光定量RT-PCR的方法检测TLR-4,HBD-2在孕妇胎盘组织中的表达,选择GAPDH为内参。结果:①宫内感染组TLR-4,HBD-2,IFN-γ的表达均显著低于HBV宫内未感染组及对照组(P<0.01)。宫内未感染组和对照组之间的差异无统计学意义(P>0.05)。②胎盘TLR-4、HBD-2与孕妇外周血IFN-γ水平均成显著正相关关系(r=0.815,P<0.01;r=0.685,P<0.05)。结论:TLR-4、HBD-2下调与IFN-γ水平下降相关,导致HBV宫内感染。Objective:To study the correlation between TLR-4,HBD-2 and IFN-γ in pregnant women with HBV intrauterine infection. Methods:57 pregnant women with positive HBsAg and HBeAg and normal hepatic function were selected as experimental group,then they were divided into intrauterine infection group(26 cases) and non-intrauterine infection group(31 cases) according to the results of two half-and-half examination of umbilical serum,20 normal pregnant women with negative HBV markers in peripheral serum were selected as control group;ELISA was used to detect the level of IFN-γ in the serum of peripheral blood;SYBR Green Ⅰ real-time fluorescent quantitative PCR was used to detect the expression levels of TLR-4 and HBD-2 in placental tissues of pregnant women. Results:The expression levels of TLR-4,HBD-2 and IFN-γ in intrauterine infection group were significantly lower than those in non-intrauterine infection group and control group(P〈0.01),but there was no significant difference between non-intrauterine infection group and control group(P〉0.05);there was a significant positive correlation between TLR-4,HBD-2 levels in placental tissues and IFN-γ level in the serum of peripheral blood(γ=0.815,P〈0.01;γ=0.685,P〈0.05). Conclusion:The down-regulation of TLR-4 and HBD-2 is related to the decrease of IFN-γ level,which induce HBV intrauterine infection.
