剖宫产后妊娠子宫疤痕破裂30例分析

摘　　要：本文对本院14年间(1975～1988年)30例剖宫产后妊娠子宫疤痕破裂进行回顾性分析,其中不全破裂13例占43.33%;完全破裂17例占56.67%。14例行子宫次全切除术(46.67%);16例行剖宫产后破裂口修补术(53.33%)。新生儿存活18例占60%,死亡12例,围产儿死亡率为40%,其中1例畸形儿,校正围产儿死亡率为36.67%。其发生率占子宫破裂的51.7%。孕产妇经积极抢救未发生死亡。作者还对疤痕子宫破裂发生的有关因素及防治措施进行论述。因此必须严格掌握剖宫产的指征,以减少疤痕子宫的发生率。Thirty cases of rupture of pregnant scarred uterus after Ceserean section during14 years(1975-1988)were analyzed,among them 13 cases(43.33%)were incompleterupture,17 cases(56.67%)complete rupture.Hysterectomy was performed in 14 cases(46.67%);and 16 cases(53.33%)had rupture repaired after Cesarean section.Therewere 18 cases(60%)of fetal survival;12 cases with one deformity of fetal death,theperinatal mortality being 40%.The corrected perinatal mortality was 36.67%.Rup-ture of the scarred uterus in the total rupture of uterus was 51.7%.No maternal deathhappened,due to great efforts made to save the lives of these pregnant women.Comments on the factors relevant to the rupture of the scarred uterus and methodsfor its prevention were made.Indications of cesarean section should be strictlyfollowed so as to reduce the rate of scarred uterus.
