超声斑点追踪技术评价尿毒症与原发性高血压患者左心室扭转及解旋运动

摘　　要：目的应用超声斑点追踪技术(STI)比较尿毒症高血压患者及原发性高血压患者左心室扭转解旋运动。方法对尿毒症组、原发性高血压组各20例,正常对照组20名,计算左心室扭转角度峰值(Ptw)及达峰时间(Ti me at Ptw)、收缩末期扭转角度(AVCtw)、扭转速度峰值(PTV)、解旋率(UntwR)、解旋减半时间(HTU),比较三组的差异。结果①尿毒症组较正常对照组Ptw、AVCtw、PTV减低,Ti me at Ptw延长,差异有统计学意义(P均<0.05),原发性高血压组较正常对照组Ptw和AVCtw增大,差异有统计学意义(P均<0.05)。②尿毒症组和原发性高血压组UntwR小于、且HTU大于正常对照组(P均<0.05)。结论采用STI技术可准确比较尿毒症患者与原发性高血压患者的左心室扭转解旋运动的异同。Objective To analyze the left ventricular（LV） twist and untwist in patients with uremia associated hypertension and patients with primary hypertension with two-dimensional speckle tracking imaging（STI）.Methods Twenty patients with uremia associated hypertension（uremia group）,20 patients with primary hypertension（hypertension group） and 20 healthy volunteers（control group） were included.Peak twist（Ptw）,time to peak twist（Time at Ptw）,twist at aortic valve closure（AVCtw）,peak twist velocity（PTV）,untwisting rate（UntwR） and half time of untwisting（HTU） were measured.All the above indicators were compared among the three groups.Results ①Compared with those of control group,Ptw,AVCtw and PTV decreased and Time at Ptw increased significantly in uremia group,while Ptw and AVCtw increased significantly in hypertension group（all P0.05）.②UntwR decreased and HTU increased significantly in uremia group and hypertensive group compared with those of control group（all P0.05）.Conclusion STI can measure the LV twist and untwist noninvasively in patients with uremia associated hypertension and patients with primary hypertensive.
