摘 要:目的:回顾性分析52例儿童发育性髋关节脱位(DDH)治疗前后的影像学特征,为DDH治疗提供诊疗依据。方法:对52例DDH儿童共70髋运用数字平片、髋关节造影、三维CT及MRI扫描多种影像学方法进行检查,采用Zionts分级标准分为4度,结合术前及术后髋臼指数(AI)和股骨颈前倾角(FNA)变化,进行手术方式的选择及术前术后的评价。结果:48髋手术治疗,术前AI(35.87±2.09)度,治疗后AI(26.32±2.89)度;FNA术前(51.38±10.67)度,治疗后FNA(19.02±2.11)度,进行统计学分析比较,术前AI、FNA和术后的差异均具有统计学意义(P<0.01)。优良64髋关节,占91%。结论:运用现代多种影像学检查,为DDH治愈率的提高提供了精确的诊治依据。Objective: To retrospectively analyze the imaging manifestations of 52 children with developmental dislocation of the hip(DDH).Methods: All the 52 cases with DDH(70 hips) were checked by digital X-ray,hip arthrography,three-dimensional CT,and MRI scan.Zionts classification standard was adopted to classify the cases according to four degrees.Acetabular index(AI) and femoral neck anteversion(FNA) were used as indicators for choosing the surgical methods and evaluating outcomes.Results: There were 48 hips treated by surgeries.The AI was(35.87±2.09) degrees before operation,and(26.32±2.89) degrees after operation.The FNA was(51.38±10.67) degrees before operation,and(19.02±2.11) degrees after operation.Both AI and FNA before operation were significantly higher than those after operation respectively(both P0.01).Clinical assessment was performed by McKay's classification.The rate of excellent was 91%(64 hips).Conclusion: Modern imaging techniques provides more informations for the diagnosis and treatment of DDH.
