摘  要:目的分析2007年和2008年9~12月昆明地区部分婴幼儿秋冬季腹泻散发病例中诺如病毒(norovirus,NoV)感染,诺如病毒和轮状病毒(rotavirus,RV)混合感染情况及其临床特点.方法收集2007年9~12月及2008年9~12月在昆明医学院第一附属医院儿科住院的腹泻患儿的粪便标本96份,其中2007年有46份,2008年有50份.用"胶体金法"检测A群轮状病毒(RV)抗原.应用反转录-聚合酶链反应(RT-PCR)扩增标本中诺如病毒核酸.结果 96份标本共有60份检出RV阳性,阳性检出率为62.5%;12份检出NoV阳性,阳性检出率为12.5%,其中5例同时两种病毒阳性,混合感染的阳性检出率为5.2%.结论昆明地区秋冬季腹泻病原以轮状病毒多见,诺如病毒次之,也出现两种病毒的混合感染.混合感染时临床症状加重,病程延长,并发症增加.Objective To analyze understand whether norovirus infections,or norovirus and rotavirus infection in infants and young children with diarrhea in autumn and winter(from September to December)in 2007 and 2008 in Kunming,and analyze the clinical characteristics of infection.Methods We collected 96 stool specimens of children with diarrhea(including 46 specimes in 2007 and 50 in 2008)hospitalized in the department of Pediatrics,The 1rst Affiliated Yanan Hospital of Kunming Medical University from September to December in 2007 and 2008.Colloidal Gold method was used to detect the antigen of group A rotavirus,and reverse transcription-polymerase chain reaction(RT-PCR)was used to detect the DNA of norovirus.Results RV infection was found in 60 children of all 96 children with diarrhea,the positive detection rate was 62.5%,and NoV infection was found in 12 children,and the positive detection rate was 12.5%.Both viruses infection was found in 5 children,and the positive detection rate was 5.2%.Conclusions Rotavirus is the most common pathogen of diarrhea in children in autumn and winter in Kunming,followed by norovirus,mixed infection of two viruses also can be found in some children.Mixed infection may aggravate the clinical symptoms,extend the course of disease,and increase the incidence of complications.
