摘 要:目的:探讨益气活血解毒法对溃疡性结肠炎(UC)复发患者结肠组织过氧化物酶增生激活受体(PPAR-γ)影响。方法:90例UC复发患者随机分为2组,中药治疗组45例,西药对照组45例。治疗3个月后对完全缓解及有效病例(其中治疗组39例,对照组32例)进行检测。治疗组给予益气活血解毒立法的中药汤剂,对照组给予柳氮磺胺吡啶治疗,并与20例作正常对照。结果:治疗前UC组结肠组织PPAR-γ水平明显低于正常健康人(P<0.01);复发组水平明显低于未复发组(P<0.01);治疗组治疗后结肠组织PPAR-γ水平明显高于对照组(P<0.05)。结论:结肠上皮能量供给下降可能是UC复发的病理机制之一,以益气活血解毒立法的中药溃结复发方,与西药柳氮磺胺吡啶比较,可通过提高结肠上皮细胞能量供给来调节免疫紊乱,从而减轻炎症反应。Objectives:To investigate influence of increasing pneuma,promoting blood flow and deintoxication on PPAR-γ expression in colon tissue of ulcerative colitis recurrence patients.Methods:The 90 cases with UC recurrence were randomly divided into the treatment group(n=45) and the control group(n=45) according to the random table.After 3 months treatment,the indicatrix was detected in the cases of complete remission and utility(treatment group,n=39;control group,n=32).The patients of treatment group accepted medicinal broth of increasing pneuma,promoting blood flow and deintoxication treatment;The control group used Salazosulfapyridine.Results:There were obvious differences between treatment group and control group.Before treatment,the level of colon tissue PPAR-γ in UC group was more lower than that in health persons(P0.01);the level of PPAR-γ in recurrence group was more lower than that in non-recurrence group(P〈0.01);After treatment,the level of colon tissue PPAR-γ in treatment group was more higher than that control group(P〈0.05).Conclusions:Colonic epithelium energy supply descend may be one of recurring pathomechanism.The study shows medicinal broth of increasing pneuma,promoting blood flow and deintoxication treatment for UC recurrence patients was more improve the level of colon tissue PPAR-γ than treatment with Salazosulfapyridine.The traditional Chinese medicine adjusts immunologic derangement through elevate enterocyte energy supply.
