摘  要:目的了解美沙酮维持治疗工作的现状,探索更为科学的管理对策。方法以47家开业1年以上的美沙酮维持治疗门诊为调查对象,采用调查问卷的方法,收集门诊一般情况、工作人员情况、后期干预工作开展情况、收支情况、多部门协调情况5个方面的资料。结果被调查的47个门诊中,有68.1%的门诊隶属于疾控中心;78.7%的门诊治疗人数及维持率未达到国家考核要求;2008年亏损的门诊占53.2%;经常为病人提供心理辅导的门诊占44.7%;与公安配合好的门诊占59.6%;有21.3%的门诊所在地2008年未召开过1次多部门协调会。共调查了392名在岗的工作人员,既往无戒毒治疗经验的占82.1%;没有接受过相关培训的占54.1%。隶属疾控中心的门诊是各项指标比较差的门诊。结论美沙酮维持治疗工作尚存在一些薄弱环节。依靠政府的支持和多部门配合、开展综合干预服务、加强培训,将推动美沙酮维持治疗工作健康发展。Objective To evaluate the current working condition of methadone maintenance treatment(MMT) project,and to explore a better management model. Methods Forty-seven MMT clinics opened over 1 year were investigated.A questionnaire,including general information of clinic,staff composition,follow-up intervention,revenue and cost,coordination of multi-sectors,was used in this survey. Results Of 47 clinics,68.1% were attached to CDC.78.7% did not finish the national indicators of the number and maintenance rate of outpatients.53.2% clinics lost,and 44.7% provided the psychological counseling service.59.6% cooperated well with police.21.3% did not hold coordination meeting in 2008.Totally 392 staff members were interviewed,among them,82.1% did not have the experience of drug treatment and 54.1% did not attend any training courses.The indicators of clinics of CDCs were poor. Conclusions There are many challenges still to be overcome in MMT project.Relying on government support and coordination of multi-sectors,developing comprehensive intervention services,and strengthening the effective training are very important to promote MMT project.
