彩色M型应变及应变率技术检测尿毒症右室心肌功能异常

摘　　要：目的应用彩色M型应变及应变率技术检测尿毒症患者右室心肌局部收缩与舒张功能。方法对照组A组30例健康人。34例尿毒症患者根据左心室肥厚与否分为左心室非肥厚组B组18例,左心室肥厚组C组16例。获取右室游离壁、室间隔的基底段及中间段的应变及应变率的彩色M型图像,并测量收缩期最大应变(maximum systolicstrain,ε)、收缩期峰值应变率(peak systolic strain rate,SRs)、舒张早期应变率(early diastolic strain rate,SRe)和舒张晚期应变率(late diastolic strain rate,SRa)。结果尿毒症B组和C组右室游离壁和室间隔的彩色M型应变及应变率的颜色较A组变浅,尤其C组显著,C组部分节段最大应变延迟;尿毒症右室游离壁及室间隔各节段的ε、SRs及SRe均为B组和C组小于A组(P<0.05),大部分节段B组大于C组,SRa在游离壁为A组<B组<C组,在室间隔为C组小于A组和B组。结论彩色M型应变及应变率能敏感、直观评价尿毒症右室心肌局部舒缩功能。Objective To detect regional myocardial systolic and diastolic functions of right ventricular myocardium in uremia patients. Methods Control group contained 30 healthy individuals,called group A. 34 patients with uremia were divided into two groups according to whether left ventricle was hypertrophic, 18 patients in group without hypertrophic left ventricle called group B and 16 patients in group with hypertrophic left ventricle called group C. The colour M-mode strain and strain rate images were acquired in basal and middle segments of right ventricular free wall and septum,and maximum systolic strain（e）,peak systolic strain rate（SRs）,early diastolic strain rate（SRe） and late diastolic strain rate（SRa） were measured. Results The colour of colour M-mode strain and strain rate was lighter in group B and group C of right ventricular free wall and septum than in group A, especially in group C whose lightness was with significance,and εin partial segments of group C delayed, ε,SRs and SRe in each segment of right ventricular free wall and septum were lower in group B and group C than in group A（P〈0.05）. In subtotal segments they were higher in group B than in group C. SRa was the lowest in group A and highest in group C in free wall, and in septum it was lower in group C than in group A and group B. Conclusions Colour M-mode strain and strain rate could evaluate regional myocardial systolic and diastolic functions of right ventricular myocardium in uremia patients sensitively and direet-viewingly.
