椎动脉血流频谱形态与锁骨下动脉窃血征分级

摘　　要：目的对锁骨下动脉窃血征(SSS)时椎动脉血流频谱形态变化进行分型,并探讨这些频谱类型与SSS分级之间的对应关系。方法使用彩色超声仪观察锁骨下动脉窃征患者椎动脉、锁骨下动脉的血流频谱形态、血流速度、血流方向及其出现的时相,同时与血管造影(DSA)结果进行对照。结果 56例SSS患者的椎动脉血流频谱形态变化大致可分为7种类型,这些类型与DSA证实的锁骨下动脉近端狭窄程度以及椎动脉反流情况相关。结论 SSS时椎动脉血流频谱的7种变化类型与常用的SSS分级之间存在对应关系,可在SSS分级诊断时加以参考。Objective To categorize the morphological changes of vertebral artery flow spectrum of subclavian steal syndrome （SSS） and discuss the relationship between these spectrum types and the Classification of SSS Methods To exam the changes of vertebral artery and subclavian artery of 56 patients who suffered from SSS by Color Doppler ultrasound, including observation of the shape of spectrum, speed,time phase, and directions of blood flow, The results were compared with that of DSA. Results The morphological changes of vertebral artery flow spectrum of 56 SSS patients could be divided into seven types. They were related to the severity of proximal subclavian artery stenosis and the reflux of vertebral artery confirmed by DSA. Conclusions There are relationships between seven types of morphological changes of vertebral artery flow spectrum of SSS and commonly used classifacation of SSS, which can be useful for the diagnoses and classification of SSS.
