摘 要:目的探讨对比增强超声(contrast-enhanced ultrasonography,CEUS)联合彩色多普勒血流显像(color Doppler flow imaging,CDFI)诊断四肢动脉损伤的临床价值。方法选取46例经CDFI诊断或高度怀疑四肢动脉损伤患者,CDFI与CEUS重点观察四肢动脉管壁连续性、管腔通畅程度及肢体远端侧支循环形成;检查结果与数字减影血管造影(DSA)或手术探查结果对比。结果 46例患者中,CDFI诊断结果包括4例可疑动脉损伤,4例部分动脉胁迫,5例完全动脉胁迫,17例动脉完全断裂,5例动脉完全断裂并血栓,5例动脉内膜挫伤并血栓形成,6例假性动脉瘤,诊断准确率为71.7%(33/46);CFUS诊断结果包括1例正常,7例部分动脉胁迫,2例完全动脉胁迫,17例动脉完全断裂,5例动脉完全断裂并血栓,8例动脉内膜挫伤并血栓形成,6例假性动脉瘤,诊断准确率为91.3%(42/46);2种检查方法相比,差异有统计学意义(P<0.05)。结论在四肢动脉损伤诊断方面,CEUS能明显增强诊断者的信心强度,提高诊断准确性,当CDFI诊断遇到困惑时,CEUS可作为首选补充检查手段。Objective To study the diagnostic value of combination of contrast enhanced ultrasound(CEUS) with color Doppler flow imaging(CDFI) in extremity arterial injuries. Methods A total of 46 cases diagnosed or extremely suspected arterial injury were examined with CDFI and CEUS. CDFI and CUES concentrated on observation the four limbs arterial continuity,unobstructed degree and collateral circulation on distal extremity. The result was contrasted with digital subtraction angiography(DSA) or operation result. Results In 46 cases,the results of CDFI included 4 cases of suspected arterial injury,4 cases of artery incomplete entrapment syndrome,5 cases of artery complete entrap- ment syndrome, 17 cases of complete rupture,5 cases of complete rupture and thrombosis,5 cases of artery intima trauma and thrombosis and 6 cases of pseudoaneurysm. The accuracy was 71.7%(33/46). The results of CEUS included 1 normal case, 7 cases of artery incomplete entrapment syndrome, 2 cases of artery complete entrapment syndrome, 17 cases of complete rupture,5 cases of complete rupture and thrombosis,8 cases of artery intima trauma and thrombosis and 6 cases of pseudoaneurysm. And the results were contrasted with DSA or operation searches results. Accuracy was 91.3 %(42/46). Indices of CEUS were significantly different from CDFI (P〈0.05). Conclusions In the diagnosis of extremity arterial injuries,CEUS can obviously increase the diagnostic confidence, and the diagnostic accuracy of CDFI. When CDFI is in perplexity,CEUS can be choosed as a supplementary method.
