胎儿颈周脐带五型分类法的超声诊断探讨

摘　　要：目的对胎儿颈周不同类型的脐带回声进行超声分型,以利掌握脐带缠绕的程度,探讨脐带绕颈的诊断标准。方法利用彩色多普勒超声技术观察脐动脉的血流矢量,对214例颈周有脐带回声的胎儿,进行缠绕方向及类型的观察。结果 214例观察对象中,绕1周者189例,占88.32%,绕2周者15例,占7.01%,绕3周者1例,占0.47%,脐带搭颈者9例,占4.2%。按缠绕角度分型:C型者76例,O型者39例,α型者90例,L型者9例,B型者0例。结论五型分类法可直观、精细地反映脐带缠绕程度,为临床提供详细诊断信息,可规范脐带绕颈的诊断。Objective To study the different echo of the fetal nuchal umbilical cord by ultrasound in order to investigate the twining degree and the diagnostic criteria of fetal nuchal circular umbilical cord. Methods We checked the vector of umbilical arterial blood flow in 214 fetuses with umbilical cord around the necks using color Doppler flow imaging（CDFI） in ordor to judge the twinding directions and types. Results Among 214 fetuses with the nuchal umbilical cord, there were 189 fetuses with one circle,15 with two circles, 1 with three circles and 9 fetuses with umbilical cord on the necks. There were five different types according to the twining degree： type＂C＂ （n= 76）, type＂O＂ （n= 39）, type＂a＂（n=90）, type＂L＂（n=9）, and type＂B＂（n=0）.Conclusions Our investigation indicates that the five types of nuebal circular umbilical cord reflect the cord entanglement degree directly and finely, which can offer detailed information to clinical diagnosis and standardize and normalize the diagnosis of the nuchal circular umbilical cord.
