摘 要:目的探讨血清脂联素对高血压患者心肌背向散射积分和左室心肌质量的影响。方法入选单纯高血压患者65例和正常对照33例。采用超声心动图Penn公式计算左室心肌质量。在AD-IBS状态测定左室心肌平均声学强度(AAI)并计算校正的声学强度(CAI)。采用酶联免疫吸附法测定血清脂联素水平。结果高血压组血清脂联素明显低于正常对照组[分别为(2.69±1.0)ng/ml vs(4.21±2.89)ng/ml,P<0.01)],而且血清脂联素水平与收缩压及舒张压均呈显著负相关(r分别为—0.274和-0.272,P<0.05)。在高血压组,血清脂联素与心肌AAI值、CAI值和左室心肌质量指数均呈显著负相关(r分别为0.46,—0.51,—0.36,P<0.05),多元逐步回归分析表明,血清脂联素能进入以CAI为因变量的回归方程中,但未能进入以左室心肌质量指数为因变量的回归方程中。结论血清脂联素水平与代表左室心肌纤维化水平的左室心肌背向散射积分呈显著负相关,是心血管疾病新的危险因素。Objective To explore the influence of serum adiponeetin level on myocardial ultrasonic integrated backscatter(IBS) and left ventricular mass in hypertensive patients. Methods 65 patients with hypertension and 33 normal controls were enrolled into this study. The left ventricular myocardium mass was calculated using Penn formula by echocardiography. The average acoustic intensity(AAI) of left ventricular myocardium were determined in AD-IBS mode,and the corrected acoustic intensity (CAD was calculated. The level of adiponectin was determined using ELISA technique. Results The level of adiponectin was singnificantly lower in hypertensive patients than that in control group (2.69±1.0 ng/ml vs 4.21±2.89 ng/ml,P〈0.05). The Pearson analysis demonstrated that the adiponectin level correlated negatively and significantly with systolic and diastolic blood pressure (r was - 0. 274, - 0. 272, respectively, P〈0.05) The serum adiponectin correlated negatively and significantly with AAI and CAI (r was -0.46 and -0.51, respectively,P〈0.05). Through multiple stepwise regression analysis,the serum diponectin was shown to be correlated with CAI(partial regression coefficient β=0. 249,P=0. 009). Conclusions The level of adiponectin correlated negatively and significantly with AAI and CAI. The decreased level of adiponectin could promote the fibrosis of myocardium in hypertensive patients and was a novel risk factor for cardiovascular disease.
