External plates fixation for the treatment of clavicle fracture:a comparing study on biomechanics

摘  要:[目的]探讨自行研制的体外钢板治疗锁骨骨折的生物力学性能,为微创手术治疗提供理论依据。[方法]采集12具成人新鲜锁骨标本,制成锁骨中段骨折模型,分别采用锁骨体外钢板、单枚克氏针和重建钢板内固定,运用生物力学实验技术手段测试各组的生物力学性能并进行比较分析。[结果]锁骨体外钢板在强度、刚度和稳定性方面优于单枚克氏针固定(P<0.05),与重建钢板内固定相比无明显差异(P>0.05)。而锁骨体外钢板固定应力遮挡小,明显优于重建钢板内固定(P<0.05)。[结论]体外钢板固定治疗锁骨骨折不但生物力学性能优越,而且应力遮挡小,优于克氏针内固定和重建钢板内固定,符合自然重建理念,是锁骨骨折一种较为理想的术式。[Objective]To investigate the biomechanics results of the self-made external plates fixation for the treatment of clavicle fractures. [Method]Twelve fresh cadaveric clavicles were prepared and arranged into group 1,group 2 and group 3 for biomechanics study.A mid-third clavicular fracture was created at the junction of the two curves of the clavicle.In group 1,the clavicular fractures were fixed with external plates,in group 2 with internal plates and in group 3 with Kirslner pins.The biomechanics capability was tested and compared. [Result]The self-made external plates provided better fixation for the clavicle fractures than the the Kirslner pins in intensity,rigidity and stability.It was similar to the internal plates fixation.The self-made external plates had smaller stress shielding than internal plates fixation. [Conclusion]The self-made external plates fixation is a good simple method for the treatment of clavicular fractures than internal plates and the Kirslner pins because of its better biomechanics capabilities.
