摘  要:目的对比多层螺旋CT血管成像(MSCTA)和彩色多普勒超声在下肢动脉硬化闭塞症诊断的价值。方法收集经彩色多普勒超声、MSCTA、DSA诊断下肢动脉硬化闭塞症的影像资料,将前两者检查结果与DSA结果进行对比分析。结果彩色多普勒超声显示下肢动脉的解剖形态和血流动力学,诊断下肢动脉硬化闭塞症敏感度、特异度、诊断符合率分别为88.04%、90.69%、88.77%;MSCTA显示下肢动脉三维结构,同时显示侧支动脉和闭塞远端的血管,诊断下肢动脉硬化闭塞症敏感度、特异度、诊断符合率分别为97.96%、96.90%、97.66%。结论 MSCTA可成为诊断动脉硬化闭塞症较理想的无创性影像学方法。Objective To compare the difference between multi-slice spiral CT angiography (MSCTA) and color doppler ultrasound in diagnosis of arteriosclerotic occlusive disease of lower extremity. Methods Patients with arteriosclerosis occlusion were assessed by color doppler ultrasound, multi-slice spiral CT angiography and digital subtraction angiography (DSA). The image information of color doppler ultrasound and MSCTA were compared with that of DSA. Results Color doppler ultrasound showed the anatomical shape and hemodynamics of the arteries of lower extremity. The sensitivity, specificity, and accu- racy for diagnosing arteriosclerotic occlusive disease of lower extremity were 88.04 %, 90.69 % and 88.77 % respectively. MSCTA showed the three dimensional structure of the arteries of lower extremity as well as the collateral arteries and the distal arterials. The sensitivity, specificity and accuracy of MSCTA were 97.96 %, 96.90 % and 97.66 %, respectively. Conclusion Multi-slice spiral CT angiography is an ideal imaging method for the diagnosis of arteriosclerotic occlusive disease of lower extremity.
