摘 要:目的研究学龄前儿童与成人眼直肌滑车连接带内胶原的差异。方法 选取10例新鲜尸体眼眶(儿童组5例、成人组5例),整体固定包埋,进行全眼眶连续冠状切片(片厚4μm)。相邻层面经Masson特殊染色后,从大体角度和光镜下观察典型层面儿童与成人直肌滑车连接带内胶原的组织结构及分布特点,并用MIAS-2000显微图像分析系统定量分析。结果 儿童与成人的眼直肌滑车均位于眼球赤道部直肌穿过Tenon囊的部位,为纤维环状结构,包绕每条眼直肌眶面,各直肌滑车之间形成连接带。儿童眼直肌滑车连接带内胶原组织形态与成人相仿,经胶原含量的平均灰度值比较(灰度数值越小,含量越多),发现内直肌-下直肌滑车连接带内胶原含量平均灰度值为74.3±0.2,成人组为89.1±0.4,2组比较差异有统计学意义(t=34.08,P<0.05)。结论 学龄前儿童与成人眼直肌滑车连接带内胶原含量间具有差异性,这一发现为更好地研究斜视病因提供了解剖学依据。Objective To study the collagen differences of rectus extraocular muscle Pulley band between the preschool children and adults.Methods Ten fresh cadaver orbits(five children,five adults)were fixed in situ and serially sectioned at 4 μm thickness.Every nearby section was stained through Masson method.After being stained,the content and distribution of collagen in the rectus extraocular muscle Pulley band of typical section were observed through gross anatomy and light microscope combined with MIAS-2000 image analyzer.Results Near the equator of the globe,the children and adult's rectus extraocular muscle Pulleys were all at the points where rectus penetrate posterior Tenon fascia,encircling the rectus orbit side by fiber structure,there were Pulley bands among rectus Pulleys.The collagen morphology of the children rectus Pulley band were similar to the adults,through collagen content comparison of the gray value(the less of the gray value,the more of the collagen content),the collagen content of the children medial rectus Pulley band(the mean gray value was 74.3±0.2)were more than that of adult groups(the mean gray value was 89.1±0.4),there was statistically difference(t=34.08,P0.05).Conclusion The collagen content of rectus Pulley band has the difference between the preschool children and adults,which provides a dissection basis for the better study of the cause of strabismus.
