摘  要:目的探索胰岛素对体外培养人脐静脉内皮细胞中C-反应蛋白(C-reactive protein,CRP)分泌及表达的影响,进而探讨胰岛素对下肢脐静脉硬化闭塞症患者血管内皮细胞的保护机制。方法取健康产妇剖宫产术后1 h以内新生儿脐带,分离并提取脐静脉内膜,用贴块法培养脐静脉内皮细胞并传代,用不同浓度胰岛素对细胞进行干预并检测其CRP和血管细胞黏附分子-1(vascular cell adhesion mol-ecule-1,VCAM-1)的变化。结果低浓度和生理浓度胰岛素组细胞生长旺盛,CRP和VCAM-1含量较高浓度胰岛素组低。结论胰岛素对于内皮细胞功能的影响呈双相:低浓度和生理浓度的胰岛素能促进脐静脉血管内皮细胞的增殖,对受损的血管内皮细胞有保护作用;超过生理浓度的胰岛素不利于血管内皮细胞生长。该结果对动脉硬化闭塞症的临床治疗有指导意义。Objective To approach whether insulin has effects to the C-reactive protein(CRP) of human vascular endothelial cell,and then approach to the protection mechanism of insulin to vascular endothelial cell.Methods The umbilical core took from neonatus was deliveried within 1 hour through caesarean section by healthy puerpera,then tunica intima of umbilical veins were seperated,endothelial cells were cultured and transferred with patch-attaching method,then cells were interfered with different concentration insulin and the changes of CRP and vascular cell adhesion molecule-1(VCAM-1) were observed.Results The contents of CRP and VCAM-1 in light concentration group and physio-concentration group were lower than those in height concentration group.Conclusion Insulin have double functions to vascular endothelial cell:light insulin concentration group can protect injuried vascular endothelial cell,and resist atherogenesis;But when the concentration of insulin is higher than the physio-concentration,it could refrain the growth of vascular endothelial cell.The experimental result has direct meaning to clinical therapy for arterial obliterans of lower extremity.
