阿托品对提高多巴酚丁胺负荷超声心动图诊断价值的研究

摘　　要：目的⑶研究阿托品对提高多巴酚丁胺负荷超声心动图诊断的价值⒚方法⑶对 ２７１ 例临床可疑或确诊的冠心病患者行多巴酚丁胺负荷超声心动图 ⒉ Ｄ Ｓ Ｅ⒕检查⒙其中 ６９ 例不能达到预估心率且未诱发心肌缺血者加用阿托品⒚２７１ 例中 ５８ 例于 Ｄ Ｓ Ｅ 检查前 ⒉后⒕２ 周内行冠脉造影⒚结果⑶单纯使用多巴酚丁胺达预估心率者 １３７ 例 ⒉５１ ％ ⒕⒙检出心肌缺血 １２１ 例⒛加用阿托品后⒙达预估心率者共 １８９ 例 ⒉６９％ ⒕⒙检出心肌缺血共 １４３ 例⒚与冠脉造影结果相对照⒙ Ｄ Ｓ Ｅ 检查中加用阿托品较单纯使用多巴酚丁胺敏感性由 ７８％ 升至 ８９％ ⒉ Ｐ＜ ００１⒕⒙特异性无明显变化⒚结论⑶在 Ｄ Ｓ Ｅ 试验中加用阿托品⒙提高了 Ｄ Ｓ ＥObjective:Atropine in associating to dobutamine stress echocardiography for diagnosis of myocardial ischemia.Methods:Dobutamine stress echocardiography was performed in 271 patients.Atropine was given to 69 patients who didnt attain target heart rate(HR).Coronary angiography was performed in 58 patients.Results:HR of 137 patients(51%)attained target,121 patients were diagnosed CAD after administration of Dobutamine alone.52 patients attained target HR and 22 patients induced myocardial ischemia after additional artopine given.Comparing with coronary angiography,sensitivity of DSE was 78% with dobutamine alone and 89% after the addition of atropine.Addition of atropine didnt influence specificity of DSE.
