右心腔不同部位起搏对犬左室舒张功能的影响

摘　　要：目的：观察犬右心腔不同部位起搏的急性血液动力学变化。方法：选用健康犬１０ 只，先后进行右心房起搏（ Ｒ Ａ Ｐ） 、希氏束近端起搏（ Ｈ Ｂ Ｐ） 和右心室腔内近心尖部起搏（ Ｒ Ｖ Ｐ） ，并测定有关血液动力学参数。结果：与 Ｒ Ａ Ｐ 比较，在 Ｈ Ｂ Ｐ状态下平均肺动脉压（ ｍ Ｐ Ａ Ｐ） 、肺毛细血管楔压（ Ｐ Ｃ Ｗ Ｐ） 和左室舒张末压（ Ｌ Ｖ Ｅ Ｄ Ｐ） 升高，心输出量（ Ｃ Ｏ） 降低，等容收缩期左室压力最大上升速率（ ＋ ｄｐ／ｄｔｍ ａ ｘ） 、等容舒张期左室压力最大下降速率（ － ｄｐ／ｄｔｍ ａｘ） ，左室松驰时间常数（ Ｔ） 均无明显变化； Ｒ Ｖ Ｐ状态下ｍ Ｐ Ａ Ｐ、 Ｐ Ｃ Ｗ Ｐ和 Ｌ Ｖ Ｅ Ｄ Ｐ升高， Ｃ Ｏ、＋ ｄｐ／ｄｔｍ ａｘ 和－ ｄｐ／ｄｔｍ ａｘ 降低， Ｔ 增加。与 Ｈ Ｂ Ｐ 比较，在 Ｒ Ｖ Ｐ 状态下 Ｃ Ｏ、＋ ｄｐ／ｄｔｍ ａｘ 和 － ｄｐ／ｄｔｍ ａ ｘ 降低， Ｔ 增加。结论：右心室腔内近心尖部起搏导致左室舒缓性下降，从而影响左室舒张功能。Objective : To observe the changes of acute hemodynamics during pacing at different parts ofright heart. Methods : Ten dogs were paced at the rightatrial( R A P) ,the proximal His bundle ( H B P) and the apex ofright ventricle( R V P) respectively ,and the hemodynamic parameters were measured during each pacing mode . Results: Comparing to R A P, H B Pshowed an increment of m P A P, P C W Pand L V E D P, and a decrement of C O. Comparing to R A P, R V Pshowed an increment of m P A P, P C W P, L V E D Pand the time constant ofleft ventricular relaxation ( T) ,and a decrementof C O,+ dp/dtmax and - dp/dtmax . Comparing to H B P, R V Pshowed a decrement of C O,+ dp/dtmax and - dp/dtmax andan increment of T. Conclusion : The right ventricular apical pacing decreases the left ventricularrelaxtation ,and there fore impairs the left ventricular diastolic function .
