摘 要:目的:研究原发性高血压(EH)患者尿蛋白的排泄情况及非洛地平对其的影响。 方法:测定30 例正常人(正常对照组)及60 例EH患者(EH组,其中包括轻度EH组32例、中度EH组28例)用药前后的尿α1-微球蛋白、转铁蛋白及血清肌酐。 结果:①轻度EH组尿α1 -微球蛋白、转铁蛋白明显高于对照组(P< 0.01),中度EH组尿α1-微球蛋白、转铁蛋白明显高于轻度EH组和正常对照组(P< 0.01;P< 0.001),且与病程有关。②血清肌酐在中度EH组明显高于正常对照组(P< 0.05),轻度EH组较正常对照组无明显升高(P> 0.05)。③非洛地平治疗后,轻度、中度EH组尿α1 -微球蛋白、转铁蛋白较治疗前明显降低(P< 0.01;P< 0.001),轻度EH组血清肌酐较治疗前无明显降低(P> 0.05),而在中度EH组血清肌酐较治疗前明显下降(P< 0.05)。 结论:尿α1 -微球蛋白、转铁蛋白较血清肌酐能更早的识别高血压肾脏损害,非洛地平降压的同时能预防或延缓肾脏损害的发生、发展。Objective:To study the changes of proteinuria in essential hypertensive patients and the effect of felodipine therapy. Methods:The concentration of urinary α 1 microglobulin,transferrin and serum creatinine in 60 essential hypertensive patients and 30 normal persons were mearured,their changes before and after felodipine treatment were mone fored. Results:1.The concentration of urinary α 1 microglobulin,transferrin in mild and moderate essential hypertensive group were significantly higher than that in the control group( p <0 01; p <0 001),the concentration of urinary α 1 microglobulin,transferrin in moderate essential hypertensive group were significantly higher than that in the mild essential hypertensive group( p <0 01).The duration of essential hypertension influences the concentration of urinary α 1 microglobulin,transferrin.2.There was no significant difference of serum creatinine concentration between mild essential hypertensive group and control group( p >0 05),the serum creatinine concentration in moderate essential hypertensive group was significantly higher than that in the control group( p <0 05).3.The concentration of urinary α 1 microglobulin,transferrin in mild and moderate essential hypertensive group after treatment were significantly lower than that before treatment( p <0 01; p <0 001),there was no significant difference of serum creatinine concentration in mild essential hypertension group between before and after treatment ( p >0 05),the concentration of serum creatinine in moderate essential hypertension group after treatment was significantly lower than that before treatment( p <0 05). Conclusion:Detection of urinary α 1 microglobulin,transferrin may identify hypertensive kidney injury earlier than detection serum creatinine,felodipine can protect and retard the occurence and progression of kidney injury besides its antihypertensive effect.
