试述公立医院公益性质和收费与支付方式改革

摘　　要：政府作为公立医院的举办者和管理者,保证公立医院的公益性质是政府的责任。政府要加大对公立医院的投入,作为保证公立医院公益性质的物质基础,同时,还应该完善公立医院的运行机制,建立科学的医疗服务收费方式和医保基金支付方式,合理补偿医疗服务耗费,并适度控制医药费用增长。文章按照深化医药卫生体制改革的总体目标要求,从公立医院公益性质的基本要点出发,结合收费方式的主要形式和支付方式的一般特点,阐述了收费方式和支付方式改革与保证公立医院公益性质之间的关系与联系,并就此提出了一些观点和建议。As the host and administrator of the public hospitals, the government should take the responsibility to ensure the commonweal nature of the public hospitals. Therefore, it is important for the government to increase more investment to public hospitals as the guarantee of supporting the welfare of public hospitals. Moreover, the government should improve the operational mechanism, establish a scientific way of charge for medical services and health care fund payment, compensate for the cost of medical services reasonably and control the growth of medical expenses appropriately. According to the aim and requirement of deepening the medical care system reform, the study mainly describes the relations between the commonweal nature of public hospitals and the reform about the charges and pay- ment way, and puts forward some suggestions from the commonweal nature, combining with the main forms of charges and payment.
