摘  要:目的:比较64层减影CT血管造影(SCTA)与数字减影血管造影(DSA)在脑动脉瘤诊治中的作用。方法:回顾性分析54例动脉瘤患者减影CTA与DSA征象,对动脉瘤的显示、数目、部位、载瘤动脉、动脉瘤形态及瘤颈情况进行分析,并提出治疗方案。结果:CTA共发现动脉瘤61个,6例患者为多发动脉瘤,其中囊状动脉瘤49个、梭形动脉瘤12个。其中1例大脑中动脉瘤DSA未显示,其它CTA与DSA诊断完全一致。12例梭形动脉瘤CTA及DSA均能清晰显示。49个囊状动脉瘤,减影CTA通过多角度多方位旋转能清晰显示全部动脉瘤形态、动脉瘤颈及与载瘤动脉关系并提出治疗意见,而DSA仅能清晰显示39个囊状动脉瘤瘤颈,其中8个不能独立提出治疗意见需结合减影CTA,差异具有统计学意义。结论:64层减影CTA简单、无创、安全,能全面显示脑动脉瘤并在显示动脉瘤颈方面具有独特优势,效果优于DSA,可作为脑动脉术前诊断的首选方法。Objective: To evaluate the value of 64-row subtaction CT angiography(SCTA) and digital subtraction angiography(DSA) in diagnosis and management of intracranial aneurysms.Methods: The image findings of 54 patients with aneurysms were retrospectively analyzed with subtraction CTA and DSA,and evaluated all the images in the number of aneurysm and the display of aneurysm body,neck,position,parent artery.Results: 61 aneurysms were found in SCTA,six patients had multiple aneurysms,of which 49 saccular aneurysms and 12 fusiform aneurysms.Including one case of middle cerebral artery aneurysm DSA did not appear,other CTA and DSA diagnosis exactly.12 cases of fusiform aneurysms were clearly displayed by SCTA and DSA.49 cases of saccular aneurysms,subtraction CTA by multi-angle multi-dimensional rotation could clearly show the entire aneurysm morphology,aneurysm neck and relationship with the parent artery and proposed treatment advice,but only 39 cases of saccular aneurysm were clearly showed by DSA,8 of which could not be combined with an independent medical advice,the difference was significant.Conclusion: The 64-row subtraction CTA is an simple,noninvasive and safe method,can fully displayed intracranial aneurysms and has unique advantage in displaying the neck of aneurysm,better than DSA.64-row SCTA can be used as the preferred method for preoperative diagnosis of intracranial aneurysms.
