摘  要:目的左旋布比卡因复合芬太尼行椎管内分娩镇痛对子宫收缩的影响。方法将75例即将分娩的产妇随机分为对照组(A组n=25)、单纯硬膜外镇痛组(B组n=25)、硬膜外复合腰麻镇痛组(C组n=25),通过临床观察和检测NO和PGE2分析分娩镇痛对子宫收缩的影响。结果 B组和C组与A组比较第一产程活跃期缩短约1h(P〈0.01),第二产程延长12~15min(P〈0.05),对产妇第三产程无影响;外周血NO水平升高,PGE2水平无统计学差异。结论左旋布比卡因复合芬太尼椎管内分娩镇痛尽管对产妇第二产程略有延长,但缩短了产妇总产程时间,子宫收缩无影响。OBJECTIVE To investigate the effect of neuraxial analgesia of combination of levobupivacaine and fentanyl on the uterine contraction in the labor process,clinical observation such as birth process,uterine contraction,active condition of postnatal recovery.METHODS 75 voluntary pregnancies were divided three groups:control group(A n=25),epidural analgesia group(B n=25),combined spinal-epidural analgesia group(Cn=25).Major steps as follows:To investigate the effect of neuraxial analgesia on the uterine contraction in the labor process by clinical observation and detecting the level of NO and PGE2.RESULTS The length of the first active stage of delivery of the analgesia groups is shortened and the length of the second stage of delivery is prolonged,The level of NO in the analgesia groups is higher than control group.The level of PGE2 in hree groups is no statistical defference in three groups.CONCLUSIONS Neuraxial analgesia of combination of levobupivacaine and fentanyl can shorte the first active stage of delivery and prolongthe length of the second stage of delivery,While it can not affect the length of the third stage of delivery.It rise the level of NO obtained from the samples of venous blood and can not affect the leve of NO from cord blood.It can not affect the leve of PGE2.
