结石性胆囊炎腹腔镜与开腹胆囊切除术的对照研究

摘　　要：目的:探讨腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)与开腹胆囊切除术(open cholecystectomy,OC)治疗结石性胆囊炎的疗效及并发症。方法:将343例结石性胆囊炎患者分为两组,220例行LC,123例行OC,观察两组手术时间、术中出血量、术后疼痛时间、肛门排气、术后下床活动、住院时间和并发症发生情况。结果:腹腔镜组手术时间、住院时间、术后疼痛、肛门排气时间、术后下床活动时间明显优于开腹组(P<0.05),术后胆心反射和切口感染率明显低于开腹组(P<0.05)。结论:治疗结石性胆囊炎LC与OC疗效相同,且具有患者创伤小、疼痛轻、康复快、美观、并发症少等优点。Objective：To investigate the therapeutic effect and complication of laparoscopic cholecystectomy（LC） and open cholecystectomy（OC） in treatment of calculous cholecystitis. Methods：The 343 calculous cholecystitis patients were divided into two groups according to the surgery method,220 patients underwent LC and 123 patients underwent OC.The operative time,intraoperative blood loss,time of postoperative pain,anus exhaust,off-bed activity,hospital stay and complications of the 2 groups were observed. Results：The operative time,hospital stay,postoperative pain,time of anus exhaust and off-bed activity were much better in laparoscopic group than open group（P0.05）.The incidence of postoperative cholecyst cardiac reflex and incision infection was much less in laparoscopic group than open group（P0.05）. Conclusions：The therapeutic effect of LC is the same with OC,having advantages of less trauma,pain,quicker recovery,better cosmesis and fewer complications.
