摘 要:目的探讨ω3脂肪酸对脓毒症胰岛素敏感性的作用。方法 30只SD大鼠随机分5组,即空白组、假手术组、模型组、ω3脂肪酸组和TPN(Total Parenteral Nutrition,TPN)组。空白组不做处理,模型组采用盲肠结扎穿刺(CLP)方法复制脓毒症模型,假手术组仅翻动盲肠,不做结扎处理,ω3脂肪酸组是加用ω3脂肪酸的肠外制剂,采用尾静脉注射给药,TPN组常规肠外制剂组。空白组、假手术组和模型组仅给予生理盐水尾静脉注射。试验期间,大鼠禁食不禁水。连续给予各组相应治疗6 d后,与第7 d 8:00对模型组、ω3脂肪酸组和TPN组进行动物造模,并继续观察16 h。常规测定各组大鼠治疗前后的体质量、空腹血糖和治疗后各组大鼠的空腹血清胰岛素水平,计算胰岛素敏感指数(ISI)。结果与空白组比较,模型组的空腹血糖和空腹胰岛素水平明显增加,ISI升高明显(P<0.05)。与模型组相比,ω3脂肪酸组的空腹血糖、空腹胰岛素水平均下降明显(P<0.05),ISI下降明显(P<0.05)。结论ω3脂肪酸能显著降低脓毒症大鼠胰岛素抵抗,增加胰岛素敏感性。Objective To investigate the effect of ω3 fatty acids on the role of insulin sensitivity in sepsis. Methods A total of 30 SD rats were randomly divided into 5 groups, namely control group, sham operation group, model group, ω3 fatty acid group, and TPN (Total Parenteral Nutrition, TPN) group. Rats in the control group were not treated with cecal ligation and puncture (CLP). Rats in sepsis group received laparotomy and cecal ligation plus punctures, and paraenteral nutrition plus ω3 fatty acid were administered into the treatment group, while the TPN group was given to total paraenteral nutrition. Saline were injected paraenterally in the blank group. Corresponding treatments were initiated 6 d before model establishment and were observed for 16 h. Body weight, fasting blood glucose and insulin sensitivity index (ISI) were measured before and after treatment. Results Compared with the control group, fasting glucose, fasting insulin levels and ISI of rats in the model group were significantly increased ( P 〈 0.05 ). Compared with the model group, fasting glucose, fasting insulin levels and ISI in rats of ω3 fatty acid group were decreased significantly ( P 〈 0..05 ). Conclusion ω3 fatty acids can significantly reduce insulin resistance in septic rats, and increase insulin sensitivity.
