肠癌肺气虚证量化辨证标准的研究

摘　　要：目的:建立肠癌肺气虚证量化辨证标准。方法:在对311例肠癌患者进行临床流行病学调查的基础上,组织专家组对其进行辨证,根据各相关中医症状在肺气虚证组和非肺气虚证组中出现状况的差异对这些症状进行赋分;应用ROC曲线的方法选择最佳诊断阈值从而建立量化辨证标准;以专家组统一辨证作为"金标准"对建立的量化标准进行回顾性检验。结果:痰白、少气、短气、咳嗽、脉无力5项为肠癌肺气虚证的相关中医症状,分别赋分为16、9、12、15、5分;量化辨证标准为≥13分;回顾性检验的敏感度、特异度、准确度均在90%以上,阳性似然比15.05。结论:本研究中肠癌肺气虚证量化标准既符合中医辨证的临床实际又较符合肠癌的临床治疗辨证的特点,且运用了比较严谨的统计学方法,相关因素赋分合理,有利于临床实际中量化标准的推广和应用。Objective：To set a quantified standard on lung-deficiency syndrome of large intestine cancer.Methods：Identify lung-deficiency syndrome by experts on the basis of clinical epidemiological investigation on 311 patients suffering from Large intestine cancer and evaluate its correlative factors on the basis of difference of their appearance between lung-deficiency syndrome and no-lung-deficiency syndrome,and set its quantified diagnosis standard and test it retrospectively.Results：White phlegm,energy deficiency,breathing hard,cough,weak pulse are its relative symptoms which are endowed with 16,9,12,15,5 points respectively;the diagnosis standard of lung-deficiency syndrome of large intestine cancer is 13 points,;the Sen,Spe and ACC of retrospective examination are all over 90% and its ＋LR is 15.05.Conclusions：The quantified standard on lung-defeciency syndrome of large intestine cancer has its characteristics：accordance with clinical characteristic of large intestine cancer and accordance with characteristic of TCM syndrome diagnosis rigorous and statistical methods is practical for its preciseness and rationality,and Clinical practice conducive to the promotion and application of quantitative criteria.
