莫西沙星序贯治疗社区获得性下呼吸道感染的临床研究

摘　　要：目的：探讨莫西沙星序贯治疗下呼吸道感染的临床疗效,安全性和治疗费用。方法：76例下呼吸道感染患者先给予莫西沙星注射液400 mg/250 mL静脉滴注,每天1次,治疗57 d。临床症状改善后,随机分成序贯组和对照组。序贯组38例换予莫西沙星片剂400 mg口服,每天1次。对照组38例继续使用莫西沙星注射液400 mg/250 mL静脉滴注,每天1次。两组总疗程1014 d。观察治疗效果,药物的不良反应和治疗费用。结果：两组的总有效率分别为86.8%和89.5%;细菌清除率分别为88.36%和89.13%;不良反应发生率分别为10.52%和13.15%,差异均无统计学意义（P均〉0.05）。序贯组的治疗费用为（3 656±386.4）元明显低于对照组（5 339±376.4）元（P〈0.01）。结论：莫西沙星序贯治疗下呼吸道感染疗效确切,不良反应少,并可节省医疗费用。Objective： To evaluate the efficacy,safety and expenditure of sequential moxifloxacin therapy in treatment of lower respiratory tract infection.Methods： Seventy-six patients with lower respiratory tract infection were treated with moxifloxacin injection administered intravenously at 400 mg/250 mL,once a day for 5-7 days.Then the patients were randomly divided into two groups by half,one（control group） was continuously given moxifloxacin injection at the same dosage,while the other group was given oral administration of 400 mg moxifloxacin tablet （sequential group）, once per day. 10-14 days alter the treatment, the efficacy, adverse reactions as well as total expenditure were compared between the two groups. Results： The total effective rate, bacterial eradication rate and the adverse reaction rate of the con- trol group and sequential group were 86.80% vs 89.5%, 88. 36% vs 89.13%, 10. 52%vs 13.15%respectively, showing no significant difference between the two groups （P all 〉 0.05）. But the average expendi ture of sequential group was significant lower than that of the control group E（3 656±386. 4） yuan vs （5 339±376.4）yuan ]（P〈0.01）. Conclusions.. The sequential moxifloxacin therapy is not only effective on lower respiratory tract infection but also an economical and safe treatment.
