子午流注蜂针治疗强直性脊柱炎疗效观察

摘　　要：目的观察子午流注蜂针对强直性脊柱炎(ankylosing spondylitis,AS)的疗效及安全性。方法将80例AS患者随机分为两组,治疗组40例用子午流注蜂针治疗,对照组40例用柳氮磺胺吡啶(sul-fasalazine,SSZ)、双氯芬酸钠治疗;两组均以4周为1个疗程,3个疗程后观察疗效,观测患者血沉(erythro-sedimentation,ESR)、C反应蛋白(C-reactive protein,CRP)、Bath强直性脊柱炎病情活动性指数(Bath ASdisease activity index,BAS-DAI)、Bath强直性脊柱炎功能指数(Bath AS functional index,BASFI)、晨僵时间、全身疼痛和脊柱疼痛Likert 4级积分、患者和医生的总体评价积分,同时观察药物的不良反应。结果两组治疗前后ESR、CRP明显下降,BAS-DAI、BASFI等各项观察指标有明显改善,治疗前后差异有统计学意义(P<0.05);治疗组和对照组总有效率分别为77.5%和80.0%,两组比较差异无统计意义(P>0.05)。但不良反应的发生率治疗组为7.5%,对照组为25%。结论子午流注蜂针治疗AS疗效与SSZ联合双氯芬酸钠相近,但其不良反应较少。Objective To observe the effect and safety of bee-acupuncture based on midnight-noon and ebb-flow doctrine in the treatment of ankylosing spondylitis（AS）.Methods Eighty patients with AS were randomly divided into two groups,40 patients in the treatment group were treated with bee-acupuncture based on midnight-noon and ebb-flow doctrine,and 40 patients in the control group were treated with sulfasalazine（SSZ） and diclofenac sodium.Both of the two groups were treated for 4 weeks as a course of treatment.After 3 courses of treatment,erythrosedimentation（ESR）,C-reactive protein（CRP）,Bath AS disease activity index（BAS-DAI）,Bath AS functional index（BASFI）,duration of morning stiffness,Likert＇ 4-grade score of whole body pain and spinal pain,evaluation of doctors and patients,as well as adverse effects of medicine were observed.Results The levels of ESR and CRP in the two groups were significantly decreased before and after treatment.BASDAI,BASFI and other indexes were improved obviously,the differences between pretreatment and posttreatment had significance（P〈0.05）.The total efficiency rate of treatment group and control group was 77.5% and 80.0%,respectively,which had no significant difference（P〉0.05）.The incidence rate of adverse effects in the treatment group was 7.5%,while that in the control group was 25%.Conclusion Compared to SSZ and diclofenac sodium,bee-acupuncture based on midnight-noon and ebb-flow doctrine has the same effects but less adverse effects in the treatment of AS.
