摘 要:目的建立一种新的、无创性的放射性核素定量检测结肠运动功能的方法。方法正常对照者32例,便秘患者24例。将Na131I密封于在胃肠道内不被消化吸收的聚氯乙烯胶囊内,受检者同时口服Na131I胶囊及99Tcm硫胶体,然后定时静态双核素采集,并计算图形中心(GC)值。结果①99Tcm硫胶体能清晰显示结肠轮廓,对Na131I胶囊进行准确体内定位;②正常对照组其口盲、结肠及全胃肠道通过时间分别为(661±194)、(3661±1051)和(4272±1002)h;便秘组分别为(803±363)、(6550±2840)和(7405±2817)h。两组口盲通过时间差异无显著性(P>005),而便秘组结肠及全胃肠道通过时间较对照组明显延迟,差异有显著性(P<001);③将检测的结肠运动功能障碍分为3型:a全结肠通过延缓型,b右半结肠通过延缓型,c左半结肠通过延缓型。结论该方法简单、合乎生理且能定量检测结肠运动功能,并能对结肠运动功能障碍进行分型。Objective To establish a new, simple and noninvasive method which can quantitatively analyze the colonic transit function by dual radionuclide colon scintigraphy Methods 24 patients with constipation and 32 normal controls were studied Na131I was sealed into capsule made by polyvinlychloride which can not be digested and absorbed in gastrointestinal tract Patients KG*4and normal volunteers swallow 131I 6capsules and drink 99 Tcm labeled surful colloid solution at the same time The static image was acquired at the regular time, then calculate the Geometric Center values (GC) Results The capsules can be clearly located through the colonic contour shown by 99 Tcm labeled surful colloid when it reached the large bowel The transitting time from mouth to cecum, through colon and through whole gastrointestinal in normal people were (661194), (36611051) and (42721002) h, respectively, in constipation group were (803363), (65502840) and (74052817) h, respectively There was no significant difference ( P >005) in two groups compared with each other But the transitting time through colon and whole gastrointestinal in constipation was slower than that in normal people, with significant difference ( P <001) Through examination the colonic transit abnormality can be divided into three patterns: whole colon transit delay, rightcolon transit delay and leftcolon transit delay Conclusions This method is a simple, physiologic and quantitative in evaluating the colonic transit, it can also stage the colonic dyskinesia of the patients
