心房纤颤对左心室射血分数正常的心力衰竭患者的影响

摘　　要：目的分析心房纤颤在左心室射血分数正常的心力衰竭（HFPEF）患者中的比例及其临床特征。方法选取HFPEF患者86例。依照是否存在心房纤颤病史或人院时心电图检查是否存在心房纤颤分为房颤组、非房颤组，并比较两组的特征。结果HFPEF患者心房纤颤的发生率为34．9％。房颤组平均年龄高于非房颤组[（71．6±8．97）岁与（62．71±17．79）岁]。多元线性回归分析表明室间隔厚度、肾功能不全和感染性疾病依次与血浆氨基末端脑钠肽前体水平相关。心房纤颤与左房内径相关性较好。结论心房纤颤是HFPEF患者常见的心律失常，左心房容积增大是房颤的主要临床特征。心房纤颤可能是HFPEF重要发病机制之一。Objective Analyse the ratio and characteristics of atrial fibrillation in heart failure patients with preserved left ventricular ejection fraction （HFPEF）. Methods 86 HFPEF patients were enrolled. Catalogue patients into two groups by atrial fibrillation history or admission electrocardiography results：with atrial fibrillation,without atrial fibrillation. Result The ratio of atrial fibrillation in HFPEF patients was 34.9%. The patients with atrial fibrillation were averagely older than the patients without atrial fibrillation（71.6±8.97 vs 62.71 ± 17.79 ,P 〈 0.05 ）. The multiple linear regression result indicated that interventrieular septum（ IVS）, renal dysfunction and infection disease correlated with plasma NTproBNP level by turns. Atrial fibrillation correlated to left atrial dimension （LAD） （ P 〈 0. 05 ）. Conclusion Atrial fibrillation is a common arrythmia in HFPEF patients. The enlargement of left atrial dimension is the primary characteristic of the group with atrial fibrillation. Atrial fibrillation could be one of the pathogenesis to HFPEF.
