盐酸米多君治疗血液透析中低血压的疗效观察

摘　　要：目的:观察盐酸米多君治疗血液透析(HD)中低血压的疗效。方法:60例HD中出现低血压的患者随机分为治疗组与对照组各30例。治疗组于HD开始前15 min服用盐酸米多君5 mg。透析开始后2h再加服5mg。对照组予以HD中禁食、高低钠序贯透析,输注高渗葡萄糖及0.9%氯化钠注射液、低温HD等治疗措施。于HD前与结束后各测1次血压,HD开始后每30 min测量1次,共10次。观察比较患者用药前后血压的变化。结果:用药后患者的血压较治疗前明显升高(P<0.01),其升高的幅度明显高于对照组(P<0.01),总有效率达到83.3%,亦明显高于对照组(P<0.05),无不良反应发生。结论:盐酸米多君治疗HD中低血压安全有效。Objective：To investigate the effect of midodrine hydrochloride on the treatment of intradialytic hypotension （IDH） in maintenance hemodialysis （MHD） patients. Method：60 MHD patients with IDH enrolled in the study were randomly assigned into 2 groups（ n = 30） ： treatment group and controlled group. The patients in the treatment group orally took midodrine hydrochloride 5 mg before the beginning of hemodialysis, and were given another 5 mg 2 hours after the beginning of hemodialysis. The patients in the controlled group were treated with cool dialysis and hypernatric ＆ hyponatrie sequential dialysis in fast or given hypertonie saline or hyperosmotie glucose. The pre-, intra-, post-hemodialysis blood pressures were measured and recorded. After the beginning of hemodialysis the blood pressure was measured every 30 minutes. The blood pressures were compared before and after the treatment with midodrine hydroehloride. Result：Compared with that before the treatment with midodrine hydrochloride, the blood pressure increased significantly （P 〈 0.01 ）. The rising amplitude was significantly higher than that of the controlled group （P 〈 0.01 ）. Being 83.3% , the total effective rate was signifieantly higher than that of the controlled group （ P 〈 0.05 ）. The side effects were not observed. Conclusion：Midodrine hydrochloride is safe and effective in the treatment of IDH.
