新生儿病房分离产ESBLs大肠埃希菌的TEM、SHV、CTX-M基因检测

摘　　要：目的了解新生儿病房产ESBLs大肠埃希菌的ESBLs的基因型。方法运用多重PCR方法同时检测70株分离自连云港市第一人民医院新生儿病房产ESBLs大肠埃希菌的TEM、SHV、CTX-M基因,并对65株CTX-M阳性菌株中的49株进行CTX-M单基因扩增,将扩增产物测序,测序结果进行网上比对,确定其基因型。结果所检测的70株产ESBLs菌株中,CTX-M单独阳性共55株,CTX-M+TEM同时阳性5株,CTX-M+SHV同时阳性5株;未检出TEM、SHV单独阳性菌株,另有5株菌未检出3种ESBLs基因;测序分析的49株CTX-M阳性菌株中,47株基因型为CTX-M-14,同时检出2株CTX-M-15。结论连云港市第一人民医院新生儿病房的产ESBLs大肠埃希菌基因型以CTX-M型为主,兼有SHV和TEM型;CTX-M型中以CTX-M-14为主;加强产ESBLs菌株基因检测对产ESBLs致病菌的预防控制有一定的指导意义。OBJECTIVE To investigate the genotype of ESBLs-producing Escherichia coli isolated from neonatal unite.METHODS TEM,SHV,CTX-M genes in 70 strains ESBLs-producing E.coli isolated from neonatal unite were detected with multiplex PCR strategy,The genotype of CTX-M genes was confirmed by partial DNA sequence analysis.RESULTS Among 70 detected strains,55 stains were CTX-M positive alone,5 strains were CTX-M＋TEM positive strains,5 CTX-M＋SHV positive trains.No TEM or SHV positive strain was detected alone.The result of partial DNA sequence analysis showed that 47 strains were CTX-M-14 type and 2 strains were CTX-M-15 type among 49 detected CTX-M positive strains.CONCLUSION CTX-M is the main genotype in neonatal unit.The major genotype of CTX-M was CTX-M-14 type.To detect the genotype of ESBLs-producing strains is valuable for the clinic medication and hospital infection monitoing.
